Monica highlights the importance of finding out from patients, ‘not what’s the matter with you, but what matters to you’.
    She also discusses the role of social prescribing in the care pathway for long-term conditions, its applicability for children and young people, and how link workers can help to connect with hard-to-reach communities to tackle health inequalities.
