    I'm a certified EKG tech and work as a cardiac monitor tech at the hospital. What I learned about the inverted P wave is that the impulse originates high in the AV junction, and the result of the rhythm or beat will have an inverted P wave preceding the QRS, and interval should be less than 0.12 secs. Also, the premature atrial contraction or complexes are premature beats that are fired out by irritable atrial tissue before the next sinus beat is due. The premature P wave may or may not be followed by a QRS, depending on how premature the PAC is. If the PAC is very premature, it will not be conducted to the ventricle because it will arrive during the ventricle's refractory period.

    Good lecture, but if you really want to be intertained, click on closed captioning and read the words. It must use voice recognition software that isn't very good. It even gets obscene at 12:50. Check it out.

    Thank you for the explanation. I have a question, though. When you are looking for WPW pattern, it is present in ALL leads? Or only in certain leads? And if present only in certain leads, can you localize the accessory pathway by what leads the delta wave appears in?

    so basically the accessory pathway can induce avrt(orthodromic, antidromic) and pre excitation pattern(delta wave)

    So first off. Thank you! Appreciate you taking the time to explain some not so easy concepts. I was reading in uptodate a sentence that said "retrograde arm of an orthodromic atrioventricular reentrant tachycardia". Since orthodromic and antidromic are new words to me, can you tell me the difference between that and Antero and Retrograde?

    Hi Doc I also suffers from WPW just hospitalised
    iin sept and oct
    but unfortunately ablation is not done in my country
    I am on medication my heart beat is around 250 BP can rise till 20

      Ablation. Catheter daalke iss abnormal tract ko khatam kar dete hain, fir koi takleef nhi hogi.

    This may be a dumb question. But is "WPW Syndrome" only confirmed if the AVRT is eliminated and a WPW pattern is seen? Otherwise, it would be as if all AVRT's would be WPW Syndrome... is that dumb?

    I had WPW until a week ago, I had it operated and removed with ablation, its GONE !
    If your hospital do the ablations, dont wait ! its relatively easy and safe procedure.
    Palpitations can be life treatining if you will be older and get any other heart problems so dont wait and remove that extra path. Technology is great nowdays.

      its so painful my heart beat reach around 250 also my BP up to 20
