For anyone who got here and is wondering "Where WAS this version used?", it's from The Trial of a Timelord, Colin Baker's final season from 1986. These titles replaced the version that had been in use since the last season of Tom Baker and throughout the Peter Davison run, finally giving Colin's era more of a visual identity of its own. Sadly, due to the attempted cancellation of the series, they only lasted for this one story (although it's actually 4 stories in one with a connecting narrative) before Sylvester McCoy took the role and the titles were redone again. You can watch Trial of a Timelord in all the usual places. I recommend getting the DVD boxed set, which has some really nice special features. You used to be able to get it on VHS in a lovely TARDIS-shaped tin (still got mine!) but that's a bit impractical. I used to make it a yearly thing to watch the entire story back-to-back, a feat that takes just shy of SIX HOURS. It's Colin's best season.
