  • 00:00 Introduction
Hey everyone, Alex from O'Brien here.
The new Straumann BLX system has many great features, but there's one aspect of the system that has confused some of our customers and in this video I'm going to try to clear up some of that confusion.
    00:18 Universal Connection?
The Straumann BLX implants all share the same prosthetic connection regardless of the size of the implant. This allows you to use the same prosthetic components across the entire implant line.
But also, that's not entirely true.
In addition to the universal components, the BLX line features some additional parts that provide extra flexibility. But those only work with the larger diameter implants.
And this is where there can be some confusion.
    00:44 RB vs WB
For the BLX line, RB is short for regular base and WB is short for wide base. The 3.5 to 4.5 implants are all considered RB while the 5.0 and above are all considered WB. The components that will work on all of these implants are called RB/WB and they feature a narrow emergence profile at the connection.
The components that will only work on the large diameter implants are called WB and they have a wider emergence that actually rests on top of the implant. Seems simple enough, but here's where it gets slightly complicated.
    01:15 Example of Mismatch
Let's say a 5mm diameter WB implant is placed and then a 5mm diameter healing abutment is used. So far so good, those are completely compatible.
Next, an impression is taken using the WB impression coping and the case is sent to the lab. The Rx that's sent with the case also mentions that it's a BLX WB implant.
The lab makes the crown using a WB abutment and then when it comes time for delivery, the abutment won't seat. So what happened?
Well, the interface of the WB abutment didn't match the interface of the 5mm healing abutment which is an RB/WB.
    01:52 Where is the Issue?
And there's two reasons why that mismatch could prevent the abutment from seating. Tissue and bone. When the RB/WB healing abutment is placed, it leaves a nice little space at the interface.
And if the top of the implant is at or above bone level, tissue is going to grow in to that space really quickly. If the implant is sub-crestal bone is most likely going to grow in to that space eventually.
So when it comes time to seat that WB abutment, you're going to have a hard time unless the tissue or bone that's holding it up is removed.
    02:21 Same Impression Copings
To complicate it even more, this wouldn't necessarily have been apparent when taking the impression because the RB and WB impression copings are exactly the same, with the exact same narrow emergence profile. The only difference is that the RB/WB impression coping is pink and the WB impression coping is brown.
    02:40 How to Prevent?
So, how is this prevented? Well, typically when you're ordering an impression coping, you get one that matches the platform size of the implant.
But with BLX, the impression coping needs to match the healing abutment instead. Whether the implant is an RB or a WB doesn't matter, what matters is the size of the healing abutment.
But remember, the impression copings are exactly the same except for the color. So, if you do find yourself with the wrong one, just make a note on the Rx about what size parts should be used and you're good to go.
    03:10 Splinted Restorations
And there's one more thing. If you want screw retained splinted restorations, the lab will need to use non-engaging abutments. And there's actually just one non-engaging option, and it has an interface that's different than the other two that I just talked about.
It's similar to the WB components in that it covers the top of the implant, but it's got a smaller diameter that works with all of the implant sizes. Due to this difference, Straumann offers specific bridge healing abutments that should be used when planning for splinted screw retained restorations.

Absolutely fantastic and useful video. I was not aware of this and had such a big problem while trying to use a 5.5mm diameter WB implant in upper right first molar region. It was my first ever case of a BLX implant, and neither did the company rep , nor the local straumann office helped me in the healing abutment selection and I wish I had seen your video first. I'll look into your other videos, and if you don't have, kindly make one on abutment selection for the straumann BL or Tissue Level (SP) implant systems. Keep up the good work.

Thnks for info first of all!
What about customized abutment ?I usually go until 4.1 We don’t need to palace 5.5 implants!
It means u want to avoid too Much the bony defects in 3D dimensional space.
And most important specially for single implant placement u would never preserve the crestal interproximall bone level…
    I think it’s a good
Implant at the end but it works only in the good minds ‘set implantologist.

Helpful but one more reason to do tissue level or hybrid implants! Especially since there is a TLX now!

I have a similar problem in the lab when creating a soft tissue model. Because of the slender emergence profile for the WB impression posts, the soft tissue ends up covering part of the platform. This becomes a problem when we get the designed abutment back and need to fit it in the model. Why, oh why, did Straumann design the impression post like this if everything else for the WB system covers the whole platform?!

Considering the decades long predictability of Straumann systems, what are the indications to even use the BLX system? Seems that it has not been thought out technically and more expensive.

Thank u for this very useful info.May I ask if instead of a RB/WB healing abutment, a WB healing abutment is used right in the beginning for a WB implant, can the above problem be eliminated? Thanks for your very very useful videos

    • @OBrienDentalLab
      @OBrienDentalLab  3 ปีที่แล้ว

Yes, if you use a WB healing abutment, that would eliminate the problem and allow you to use any of their prosthetic components (WB, RB/WB, Bridge) without an issue. Just remember that the WB components can only be used on the 5.0, 5.5, and 6.5 implants.

Hi Dr. I am a patient and your opinion is very helpful to me. I had two implants Straumann BL Roxolid SLACTIVE size 4.1* 10 mm In the lower jaw less than a month ago and still waiting for screw retained crown procedure, my question which is the most appropriate healing abutment which I should use or rather should be used by the lab? Waiting for your reply. My deep thanks and gratitude.

Your doctor would need to decide which healing abutment is most appropriate for you

    Hi There,
So I am going to be restoring a Straumann BLX 5.5 x 10 mm which currently has a 6 x .75 x 4mm healing abutment present, what will be the best impression coping to use?

    • @OBrienDentalLab
      @OBrienDentalLab  ปีที่แล้ว

That is a WB healing abutment, so you will want to use a WB impression coping. Closed tray: 065.4810. Open tray: 065.0032 (short) or 065.0034 (long). You can find them at this link: shop.straumann.com/us/en_us/Prosthetic-Solutions/BLX-/For-Wide-Base-%28WB%29-Implants/Impression-Components-WB/c/cat_stmn_imprcomponents_wb
