Epstein Barr Virus and Infectious Mononucleosis (pathophysiology, investigations and treatment)

    "EBV causes typically an asymptomatic infection or can cause acute tonsillitis as a part of bigger infection called infectious mononucleosis also known as glandular fever. EBV typically occurs in adolescents and young adults and is transmitted orally via saliva. This is the reason EBV infections are also known as the kissing disease. Nearly everyone will become infected with EBV at some point, the good thing is for most of us it is asymptomatic. In this videos we will talk about a primary EBV infection which is asymptomatic and how the infection can evolve to be become infectious mononucleosis which is a symptomatic condition.
    The pathophysiology of EBV infection begin with saliva transmission and the EBV virus targeting the tonsils which is a lymphoid tissue made up of T cells and B cells. In the tonsil the EBV target B cells and tonsillar epithelial cells. This is called the EBV primary infection and is usually asymptomatic. Once EBV infects the B cells the EBV has an Incubation period of about 2-6 weeks. During the incubation period a number of things happen:
    EBV replicates in B-cell and the virus is shed intermittently into pharyngeal secretions, particularly saliva. Saliva is therefore how EBV is transmitted between people.
    The cycle can continue and the person can be completely asymptomatic of course during this incubation and reactivation time the immune system will respond.
    EBV Induces an immune response where by b cells capture an antigen of EBV and processes it, The b-cells can mount an immune response locally in the tonsils and the B cells enter circulation and mount an immune response in the lymph node, spleen and liver. Here the B cells stimulate CD8 T cell activity. CD8 t cells also known as cytotoxic t cells are important in the suppression of primary EBV infection. On a side note the CD8+ are thought to be the important players in preventing EBV reactivation and EBV associated lymphoproliferative disease. The B cells also activate CD4+ T cells through costimulation, which means the B cells also become activated. Activated B cells become plasma cells. The plasma cells are the antibody producing cells. The plasma cells will produce EBV specific antibodies which mean antibodies against components of the EBV - firstly the viral capsid antigen with IgM followed by the viral capsid antigen with igG. Plasma cells then eventually produce epstein Barr nuclear antigen igG once infection is resolved. The activation of other immune cells in the lymph nodes, spleen and liver is part of the immune response against EBV.
    During the incubation period the EBV can also enter circulation. Here the immune cells in the blood will try to destroy them and also mount an immune response releasing cytokines. This interaction is one of the reasons why people in EBV infection become febrile and feel miserable.
    During the incubation period the abnormal infiltrated B cells produce heterophile antibodies which is an important and quick marker for diagnosing EBV"

    This is by far the worst sickness i’ve ever undergone. I had a severe sinus infection on top of it. Couldn’t eat for 7 days lost over a year of work in the gym my body turned to noodles. The sore throat lasted 16 days for me. Coughed up an unbelievable amount blood and mucus. Fever got up to 105.9 degrees and lasted probably half the day anywhere from 101-103 degrees usually. The fever didn’t affect me honestly. The sore throat was unbearable, to the point where my nose closed up becaude of congestion, then i had to breathe through my mouth, my throat completely closed up during sleep and i started choking and forcing water down my throat to frantically try to open it up or my nose and after a few seconds of panic i was able to. I’m only 19 and i was at the height of my health. I am the healthiest in my family. This shit is something other wordly. Covid wasn’t shit compared to this. i was put in the hospital for 13 days because it was so severe. My spleen was so inflamed every time i swallowed anything it was so bad that my whole body jumped and that caused my spleen to hurt every single time. This was created by the devil himself. i’m still trying to get over this shit. God bless anyone who has this right now.

      Im have this shit right now. My symptons are the same but not as severe as yours were, still im frightened of the nights cause the nights are the worst im crying and sometimes halucinating due to lack of sleep and the constant throat and ear pain. Im crying so much..I swear this can make the hardest atheist into a believer lol. Sry for bad english.

      I got it 14 and i coudnt do anything was on the bed for weeks and couldnt eat for 2 weeks and it was indeed for me aswell my worst expirience and beacuse of my throat i couldnt breathe at night and wasnt able to sleep beacuse i woke up from almost suffocating so had to get special equippment to help me breathe and much more

    It brings me comfort the we are all suffering together here. I haven't slept in 3 days due to the pain. I have a diet of broth and popsicle sticks right now. I am a freshmen in college and am falling severely behind in school. WERE IN THIS TOGETHER GUYS AND WE WILL SURVIVE AND NEVER FEEL THIS AMOUNT OF PAIN AGAIN!

    Undoubtedly one of the worst and most painful things I’ve ever experienced. It left me with a physical exhaustion that lasted much beyond the tonsil swelling, pus, and burning throat pain. Worst part was that I caught it during a time that I wasn’t kissing or coming into contact with someone’s saliva. Best I can assume was that it came from eating out while abroad (I started presenting symptoms a bit after coming back from South Korea and Japan. My girlfriend at the time stayed back in America so I obviously hadn’t been kissing anyone and she never ended up catching it.)

      Same happened to me back in January!! I had severous tonsillitis, couldn’t even pass my saliva, fever and the white patches, 3 months after I still feeling fatigued and sun exposure makes my tonsils swell a little. Do you know if is it normal to still have a high count of white cells after 3-4 months of the infection? My recent blood test came out with high lymphocytes and I’m worried😢 I have no other symptoms tho, it’s just the high white cells count

    I had mono when I was 18 and it was the SICKEST I’ve ever been in my life. I had a 104 degree fever for several days, EXTREME fatigue for 2 weeks (I could not stand long enough to take a shower), body aches, constant vomiting and strong aversion to food. Just thinking of food made me gag. I had strep throat at the same time. The scariest symptoms I had were jaundice (my eyes were very yellow too) and brown urine. The doctor was very concerned about my liver as it was highly inflamed. I lost a lot of weight and finally recovered at just 95 lbs and I’m 5’5!! Honestly, I would rather have just about any other virus than this again. I really thought I might die.

      Jeffrey Bozko I’m quite sure from sharing drinks while at tennis practice. Despite the nickname, it’s rarely caught from kissing. You can get it by sharing utensils, food, drinks, not washing hands, walking through droplets from where someone just sneezed or coughed... very young kids get it from putting everything in their mouth. It’s very easy to catch and that’s why 95% of adults carry the virus.

      Padmini Taikram Unfortunately there is not much you can do other than take ibuprofen and rest. Drink lots of fluids, and try to keep your body temperature down. Force yourself to eat at least once a day. You will recover faster if you can make yourself eat. Good luck to you! You have my sympathies!

    Thank you SO much for this video. I understand most of it, You were really taking me back to the beginning of Nursing School. I am currently in a state of a reactivation phase of the EBV. I have luekopenia at 2.2 and my tonsils are currently swollen with fever and headaches, achy joints and severe fatigue. I have had my homocysteine levels checked out elevated as well as my Methymalonic Acid serum at 1062. What's horrible about this is that it's not something you can see. There is no sympathy for a person who is stricken with EBV. I certainly appreciate this video. Maybe I can show it to the naysayers. Thank you for your insights into the life of a person undergoing an reactivation of the EBV and how it affects one's body.

      Hello Ella,
      I cannot diagnose you as I'm not a Dr. BUT, if I were you, I would see a Dr. of Hemotologist/Oncoloy who can reassure you of what's going on. I had a huge amount of testing done to find out what's going on before I had a diagnosis. I would start with a journal of your symptoms while awaiting an appointment with a Dr. You might need a referral so you might need to see a Primary Dr. first.
      Start your journal with when it started, how long it lasted, how did you feel, the same as this time or worse? Start from you head to your toes including if you have Athletes feet! Remember everything! Exclude nothing! Nothing should be ignored including your stress levels. Make sure that you include what you eat, meds you take, even recreational habits. Do you have a Family history of anyone else with these same symptoms? Who have you come in contact with? It's imperative that you start somewhere so that they can help you best. I'm so sorry to hear about all of those terrible symptoms. Start with warm salt water 4 times a day, swishing in your mouth to ease some of the pain from the sores. If you can tolerate them, you might want to try Zinc lozenges as well for some slight benefit. Best of health to You. Start writing as soon as possible. Take all your notes and write on your own body if necessary with a non toxic pen if there's something that hurts! I've done it. Your not crazy, but you do need to see a doctor asap. Start journaling to get out of your head. It's actually cathartic and gives you back some of your power. Take good care of yourself. Let me know how you make out. ✌
      PS~If your white blood count is low, your more susceptible to getting sick. You have to see a Dr to first to determine that diagnosis. Some people do live with low WBC all their lives. I'm not one of those people. I have a normal range between 5-10 thousand so I can't tell if you have lucopenia or not. ✌

    Currently going thru this. My first symptom was unbelievable spleen pain and then slowly slowly i developed the rest of the symptoms. I wake up in panic every time i try to sleep, gasping for air as my tonsils r so enflamed that my airways r nearly completely blocked and my nose is so congealed with mucus. Ive had symptoms for a week. I hope it gets better from here.

    It took years for me to get back to somewhat normal after having this over 30 years ago. My liver became inflamed and the doctor originally thought I had some type of viral hepatitis. The bloodwork showed that it was EBV. It got so bad it felt like I had a mild stroke. My entire body had gone numb, especially my extremities. My reaction time had extremely slowed down, also. The malaise turned into debilitating depression. Several years later, the autoimmune problems started, such as IBD and then inflammatory arthritis.

      @@onetothree228 I would say maybe 7 or 8 years. You might want to consider getting on antidepressants. They might help a little, but have terrible side effects of their own. It's a terrible situation to be in.

    Hi Armando! Are you available to answer a few questions?
    When I was 4 yrs old, I had acute tonsillitis and had my it sinks removed. Now as an adult, I have ms, with a low CD8 T cell blood count and an elevated CD4/CD8 ratio. Do I have a lot of CD8 cells sequestered in my CNS doing the damage and that's why they appear low in my peripheral blood? Or do I genetically not reduce enough CD8 cells and that's why my body is unable to fight the EBV, hence the ms? Thank you.

    Had to go to the hospital for this. I almost definitely got it from my girlfriend that I met up with for the first time after 2 years of long distance contact and made out on many occasions whereby transmission was all but assured. Took 3 weeks for symptoms to begin but it began with a feeling of eye soreness in the periphery of my vision, I couldn't understand it. I worked the entire week regardless. The next week I was not able to work as symptoms began to progress further towards a full-blown sickness. I don't even remember the timeline of the events that unfolded anymore but I think I got fever in that week, I ended up going to the ER clinic near me. I rejected the doctor's recommendations to go to the hospital as I thought it could be resolved from home. At this point I was now taking a second anti-biotic which we'd find was not useful at all in combatting the infection since no one knew what it was yet, somehow. My lymph nodes began to swell, my throat began to feel pain, and I eventually started having fevers up to 103 F. Horrible restless nights and bed-ridden days. My parents and I decided to go to the ER again and this time I was transported to the hospital on a case of sepsis-alert. For the next week I was placed on an IV of saline and antibiotics straight into my veins and this also proved to be a very very unpleasant time. One night was particularly awful in that I could not possibly fall asleep and this tormented me for the duration of the entire night. Had my blood drawn probably 10 times in this period and eventually the problem was discovered as being the Epstein Barr virus. The fear of having fever is still one that keeps me on my toes, even though I know that time is past. I had fever for nearly 4 days in total. Now after being released I've spent 7 days at home with a feeling of imbalance and this sort of generalized malaise that was discussed in the video. I feel much better but still not great as I approach the 4th week of this illness. Oh and swallowing, wow. The feeling of swallowing at the peak of my tonsillitis was as if though you were grabbing a sharp knife and ramming it straight into my throat everytime I'd swallow. I couldn't even eat. I needed steroids and pain relief just to eat, which was put through IV. One of the worst experiences of my life.

    Very interesting thank you! I got it years ago and had a really bad case, turned chronic 😪 my doctor didn’t really know so first two times I came in I got antibiotics only after 2 weeks she told me it may be ebv.. I was then super sick for a month, then moderately sick for 2 years and now 8 years later still cf and other problems 🥺😭🤯
    does this infection impact the way my body responses to COVID-19?
    Only later on they did blood work and much later more proper blood work and I had antibodies but also found out I had hepetitis infection on top..

    I have MONO now, tested positive and I’m 61 years old. I am single and have everything delivered to me, so I’m not sure how I got it. I’ve been sick with a severe burning throat, extreme fatigue, and mild to moderate hoarseness. I started showing symptoms on February 16, 2023 and it’s now March 10, 2023 and I’m still sick. Interestingly, I never got any rashes, no fever.

      I too have suffered with major fatigue and flare ups and was told I have Chronic EBV, as well as Fibromyalgia, Chronic Fatigue Syndrome etc....since 2006 when I first became ill with EBV

    I believe it’s also related to herpes virus,because after I went to wedding,a friends ,were shaking hands ,strangers kissing me , ahhh next time 2 days later,they have a huge cold sore . Coincidence? I think not ! I get my first cold sore next month and mono ,I had mono 3-4 times as a kid, and severe tonsillitis, I had to get them out ., but what triggered it back,other than stress ? I know my body,and I think the two have to be related. Again there’s many different types,so I need to dig deeper. We also need to help others because this is related to CFS,and it’s hell..

      TheNamelessName Yeah I mean I was old enough,to know that I’d never had a cold sore in my life . However after that wedding,and strangers kissing you , even on both sides of your cheeks, it wasn’t that long after m when I saw 4 specific peonies that lineup with cold sores .
      I had my first one 7-9 weeks later. It’s hurts . To this day I wonder if it triggered my CRPS.

    Mono sucked for me. Luckily I got it when I was young. I felt like all I did for a month was go to work followed by going home and sleeping until it was time to go to work again. I guess it wasn't too bad I just had swollen lymph nodes in my neck but the constantly being tired was very hard after a couple weeks.

    Hey so I have a question I want to see if someone can answer. My mom was diagnosed with ebv in 2015. I don't remember her having symptoms. I know I probably did drink from her I'm sure sometime. I never had mono nor have I been diagnosed with ebv. I also have never had the symptoms of ebv so is it possible I have it? If so should I watch out for my spit and blood? I'm washing my hands constantly and I'm always worried about infecting others. Should I be worried or not?

      Most of cases of EBV are asymptomatic, so it is possible to have EBV even if you are was symptoms-free .
      EBV stay latent in B cells and you stay infectious months after recovery, because it transmitted by saliva mainly which is normally produced .,
      All of us are already infected with that virus, so you do not need to worry about your infectiousness .

    Very interesting thank you so much for this video it makes a lot more sense now with all the symptoms I have had since I was a teenager after having mono that has now turned into multiple sclerosis with swollen lymph nodes. 😖it would have been helpful to understand all this a long time ago so that I could be very careful about my health and do everything I could to prevent the EBV virus from reactivating but I had no idea that it could reactivate and that it may have something to do with multiple sclerosis I am understanding now based on several medical investigations.

    I had ebv and then after a very stressful period of time in my life it re-activated. I developed hashimotos hypothyroidism and then sibo/candida (along with chronic fatigue and pain). 5 years of my life has been ruined by this.

      I have Hashimoto thyroiditis as well. I am unsure if its mono or not but doctors says it is mono. I have been to ER mutiple times. Don’t know if i need to go another hospital too. I have no improvement in my symptoms. Its been more than 3months. I think that i have throat or neck cancer, its so depressing. I don’t know how long it takes for people to recover from this but in my case, everyday, i wake up with a new symptom from march 2024. I still have swollen lymph nodes and inflammed pharynx. The only thing i dont have yet is fever and weight loss. I don’t know which doctor should i go to! I dont know if even the doctor is really truthful towards diagnosis in canada. This is very depressing for me. I have lost all hopes.

    Excellent video demonstration. Very informative to clearly understand. I had mono in HS, and since 2009 I’ve had a blood test each year that has come back positive. I suffer daily. I can’t seem to come out of it. I was on a antiviral for a year and didn’t help. I’ve taken every vitamin under the sun it seems. That have given me a good hour here or there or so. Right now my levels are 600. And that was when I could finally feel good enough to make it out of the house and see doctor. Any suggestions would be appreciated. Thank you in advance.

      Hi, thanks for sharing that. I'm doing the Celery Juice treatment and I THINK it's helping though I still have flare ups. Overall feeling better over last 5 weeks of trying this. Will get some wild blueberries. Eating tons of and growing sweet potatoes. What do you think about grains? (Which I love...)

      @@ohialani It's all about the quality got he grains isn't it. If it's truly organic than your stomach might handle it. I went off bread and eggs for a good while while I was healing which seemed to help a lot but now I eat sourdough bread again since its supposed to have a lower gluten content.
      Grains are tricky though I haven't quite figured that one out myself to be honest. 🙏

Losing weight stop smoking go off all sodas including diet sodas don’t eat any fast foods and cut up all gluten breads crackers. If you change your diet you’ll see your Health turn around. But it’s up to you no one can do it for you. I always like to say if your grandparents or your great grandparents didn’t eat it you shouldn’t either. Don’t ever eat out of a box. Cook and prepare all your own food. Avoid white potatoes, white bread, brown bread, all breads, avoid rice.
      At the very least do all of this initially to turn your Health around. Cheat days are always allowed but not until you’ve gotten your good health under control.💪🏼👍🏼🍀

      Man.... I think I may have this. Constant muscle spasms in all areas of the body. But now months later only really in legs and sometimes stomach.       My daughter got ebv cause lung , liver and dermatomyosist. Dr gave her prednisone to help her dermatolmyosist, which reduce her immune system, allow the ebv to grow and destroy her other organs. She died 2 days after taking prednisone

      You only get IM (infectious mononucleosis) once, during primary infection. Your throat problem is probably a recurrent bacterial infection. After first infection with EBV (asymptomatic during childhood or acute IM during adolescence/adulthood), you develop immunological memory against EBV so don't develop the symptoms of IM again next time you encounter EBV. Although the EBV is never completely eliminated because it becomes latent in your B cells, it can never cause IM symptoms again.

      @@Nk51188 This is one of the most false statements I’ve ever heard, and you’re trying to sound educated. Do your research on chronic EBV. Yes, for the large majority of people it stays dormant in the body- but it can absolutely reactivate and cause IM symptoms particularly in people with weakened immune systems and become a chronic issue. Don’t spread false medical information. There are hundreds of thousands of people living with chronic EBV, including me. I’m on my third reactivation and it has lasted so far 3 months.

    I had glandular fever back in 2015. For a week i had a lump on my throat, Then followed a cough, fever and vomiting. My neck was the size of a rugby ball, im not joking. I could not drink or eat without vomiting. My throat was BURNING,I was breathless, and my muscles were on fire. I ended up in hospital for 3 or 4 days.
