Bertholt was actually supposed to be the best amongst them (Them being reiner, Annie and crew)... He was just quiet and low key. That's the reason he was able to hold his own against Mikasa. There are a bunch of small hints throughout the story so far.

I personally watched aot when I was 15 and I didnt understand shi, I didnt know who Moblit is, who Marco was, who Marlo was, all those side characters I kinda forgot about them and didnt understand like half of the story. Only upon rewatching 2 times and reading online posts about aot I rly understood everything and now I know every detail.

Erwin was enslaved to his "dream" and cannot find peace for as long as he lives, only death was there liberation. That's why when it flashed back to their conversation, and Levi told him to sacrifice himself with the troupes, Ewin smiled and said "Thank you". He released him from his "hell". So Levi made the choice not to bring him back.

Intentionally or not is up to readers interpetation. Could Erwin be simply reliving the memory and raised his hand in the exact second Levi was about to inject him by accident? Yes. Could Erwin also be reliving that memory but at the same time hearing the whole conversation and getting last of his strenght to lift his arm up so Levi dosnt inject him? Also yes. Anyways, in both cases it was Erwin that accidentaly or not turned it down. If Erwins hand didnt move Levi would inject him.
