I reckon due to how we see the monster, which takes the dog (it looks like him) and how relatively unfazed he is by it all, the boy imagines the smiling face and dreams about all of this because he's a bit fucked up. Looking at how he draws the face at the end and the ways in which he loses the items he has to get makes me think the smiley demon is just a manifestation of something in his mind. We also see a possibly older version of them lying on a carpet at the end hinting this is all memories.

I think the story is not difficult to interpret. It is the story of a man who has died in ruin and in a deplorable condition both physically and mentally. I think that all this that we see of him as a child are simply the manifestation of his wishes and his happy memories as a child, although apparently it is not that he had a very happy and welcoming childhood, but as it is the closest thing to the best stage of his life, he remembers it and wants to live in it eternally. However, something inside him is not right and it constantly haunts him, he always wants to return to that peaceful stage of his life but something never leaves him. And all this we see I suppose are the last moments of his life, a world that was created by his brain in his last breath. In the end, in his mind he always lived in constant torment and deep pain that are reflected in these strange mental constructs.
