Love this! I started a form off your sample & was thinking I would add a box under the date with my basic lesson plan/activity & just submit that info ahead of time for multiple days. I toggled the "edit responses" ON in the settings, but I can't seem to edit the form itself to enter the data on a later date. I can only edit a form just after I submit it under the "your response has been recorded" prompt or link to the Google Sheet & enter information there. But on Sheets, you can't enter data like you would on the Form (e.g., marking the checkboxes,...) and any text info I've entered in Sheets does not "link" back to the responses summary nor does it update the graphs on the form.
Any thoughts? I suppose I just need to keep a separate sheet for lesson planning, but was just trying to consolidate it all to one form/location for each student to decrease the paper or file shuffling...

Let me tell you that you are a brilliant SLP! Thank you for sharing this step by step. This is how my brain works. The only challenge would be to make these sheets for 55 students.

Lol! This student has a TON of goals! Seriously though - this is amazing thank you for demonstrating how to do this- I didn't know how to utilize google forms for speech data collection before seeing this. Thanks! Could you do a video on how you organize this in your google docs and how you access in the drive?

Hi Katie! Haha yes that poor student has so many goals. Luckily they aren't a real student. :) I made a quick little video to try and answer your question. Hopefully, you find it helpful! CLICK HERE TO WATCH: www.loom.com/share/50091095bbbd48d9bded80a81ec92556
