venoms bite has to requirements #1 be in the green zone or #2 be poision having any of thse requirements causes him to grip you into the air and die though possible to dodge by dashing in the air (According to final) I doubt it

How do u dodge the phase 3/4 attacks like for the sword slash combo which goes up into the air?

Unobtainable, but it will probably be obtainable soon.
it used to be obtainable by defeating Lord Vetus and staying alive.
