When you are cutting hold your handle with slight pressure toward the base.

Hole puncher brand is RAPESCO & edge puncher I got it for free on an order, but I have mine from Michaels size 5MM. Any brand will work!
Check you blade on you paper cutter, it may have a nick in it in that one area. Mine did, I had to buy a new blade.
