For whom the bell tolls, time marches on, means that they are marching towards their death. The bell is tolling for them, like it is calling to them, summoning them.

The classic saying that Hemingway borrowed for the title of his book goes : "Send not to know for whom the bell tolls - it tolls for thee" (variously paraphrased). This is from an even older work by John Donne where the saying is meant to remind us that we're all going to die and that the bell tolls for all of us. So when Hetfield here says "time marches on", he's referencing that meaning of the phrase.

I think what you’re confused about is It’s not just “for whom the bell tolls” the lyrics goes “ gone insane from the pain that they surely know For whom the bell tolls. Time marches on.” You’ll understand it if you read the lyrics as a whole not just a random half sentence.
