Hi, fantastic video. I like your methodical and thorough approach, I recommend your channel to my students! Could you please do a video on temporisation of crown/veneer preparations? Thanks
Thank you so much mentor! Could you please tell me what is the right (or perfect) working position and the position of the head when doing lingual reduction on dental simulation head.
doc could you advise how to figure out if the handpiece or the bur is not working optimally?? the frequency of changing the bur or handpiece and the optimum cutting speed?
I change diamond burs after 3-5 preps. If cutting dry, even more often. The handpeiece should be lubricated daily. and when you turn the bur with your fingers it should move freely with no resistance. Other than that, keep experimenting to find your zone...
Thank you doctor for your efforts to teach us. But I've a question... What about using a wheel diamond stone to reduce the palatal surface including the cingulum before or even after the tapered stone with flat or rounded end to draw the finish line?!!!
This is so helpful, thanks for posting! Do you by chance have any tips for finding the long axis of the canine when doing this prep? That’s where I struggle the most- thanks!
Try to achieve 91-110 degrees with respect to the root or unprepared apical tooth surface on the facial finish line. You don't want this to be less than 90 degrees, as this would leave unsupported tooth structure.
usually if you prep to the tissue level and reduce 2.0 mm from the incisal, you'll have enough length - it will vary between typodonts and clinical practice, but 3 mm vertical walls mesial and distal would be sufficient.
It’s all about line of draw. Since the teeth don’t naturally line up, you’ll need to taper the distal of the canine more than if it was a single prep. Also, make sure that you have even shoulders of adequate depth. Making the preps smooth is good but not as important. Best wishes!
Hello doctor. In Australian Dental Council exams we have to prepare 1.2mm shoulder. When I do that I tend to get more cutting from proximal areas. Is their a criteria for proximal cutting? Or as long as the taper is fine and margin in transitioning it's ok?
As long as the measurement is 1.2 and the taper is correct, the results wil be good. If you can use a more narrow bur at first, like a 850-014 this might help...
Why did you not use end cutting bur, instead using it at the end? I found that, when I use end cutting bur while doing finishing line on buccal surface, my preparation tends to have equigingival shoulder on line angles.(any suggestions to improve on this plz)
The end cutting bur is potentially very dangerous, as it can cause internal undercuts. It should be used to simply refine the shoulder carefully. Keep the finish line 1 mm above the tissue before refining - this way, the end result will be 0.5 mm in most cases.
PFM: Facial Shoulder 1-1-2 mm deep and relatively sharp internal line angles, and minimal reduction on the lingual with a chamfer margin. ACC (e.max): 360 shoulder with rounded internal (aka fillet) which may measure between 0.5 for e.max in the most conservative cases to as much as is needed for esthetics, but typically about 0.6-0.8 mm. MONOLITHIC ZIRCONIA: Same as ACC but must be at least 0.8 mm deep axially, but may be less on the occlusal.
Great questions: If you have an electric handpiece, turn it down to 2500 and use a finishing diamond to smooth the finish line. Otherwise, use stall-out speed or even a slow speed fitted with a friction grip attachment to refine. Correcting uneven depth takes practice but try using the bur tip to define it more uniformly - the tip of the 8847-016 is 1.1 mm. Another great bur to try is Brasseler's 8881-012 - Best to you, Dr. S
@@StevensonDentalSolutions Thank you so much for your kindness. I use only a high-speed handpiece, not an electric handpiece. Is it possible to prepare teeth with FG low speed handpiece? It looks like you're using a fine bur(red band).
Good work doctor.. Plz can you tell me where i make bevel in all teeth.. iam still astudent .. i know only the bevel in the lower molars in the buccal cusp and the upper molars in the palatal cusp .. Is there abevel in the upper and lower premolars and the upper and lower anteriors?
For posterior teeth, bevel the functional cusps. For anterior teeth, simply reduct enough for incisal translucency, facial ceramics and lingual clearance for function. I'd recommend that you carefully watch the PFM and FGC playlists.
@@StevensonDentalSolutions thanks doctor.. So the bevel in the upper premolars is the platal cusp and the lower premolars is the buccal cusp as the molars.. is this right?
@@StevensonDentalSolutions enamel hatchet and gingival marginal trimmer plus for interproximal I always end up with over tapering.. Any tip for that please... Thank you for your reply.
Can u write the measurement of prep pleas Iam know from incisal 2mm From lingual clearance 1.5 mm and know from facial 1.5 mm whu u take 1mm??? and from lingual axial what is the measurment please
The final prep: 2 mm incisal, 1.5 mm facial at incisal 1/3 and 1-1.2 mm at margin. 1-1.5 mm lingual clearance and 1-1.5 mm lingual reduction in fossa. Lingual axial wall is 0.5-0.7 mm (chamfer). I start with less and finish to the final measurements to avoid over-cutting.
Keep a thin shell between the axial wall and the adjacent tooth - use a thin bur like an 859-010. After breaking proximal, you can progressively increase bur diameters...
847KR-016 is considered a medium grit and has 100 micron diamond particles, and has no color ring on the bur shank. The 6847KR-016 is a coarse grit and has 125 micron diamond particles and has a green ring around the shank. The 5847KR-016 is an extra-coarse grit and has a black ring around the shank. The finishing diamond in this shape (the *847KR* shape) is the 8847KR-016, with a red ring on the shank with 30 micron particles - a fine grit. I use the 847KR followed by the 8847KR, however it works equally well to use the coarse 6847KR instead of the 847KR.
