4. IELTS INSTITUTE AND ONLINE FOOD DELIVERY BUSINESS: Also ran my business for more than 1 year.

For the employment letter and education letter, mine both did not come with contact point of my supervisor(s). My question is: should I make an additional document to write the contact number? I have one question, there are 2 copies of form 990A in the the pdf at the end of the email only has 4 pages while on the link it has 8 pages which one to fill

Cathay Pacific had taken my TOEIC Listening & Reading Text online but CX hasn't shared the result, I got the invite for the physical Interview in Mumbai. My query is that-
from the list of documents that I will be bringing on the assessment day, it is asking for the scores but I don't have it. What should I do then?

We are asked to fill few forms For the final assessment to be held on 5th 6th and 7th of FEB and this is one of it.
I am not sure how to go about it for the dependant section.
