It annoys me the press continually go on about the efficacy number. Frankly who cares - if after 21 days you don't end up seriously ill or in hospital its job done as far as I'm concerned. I'll take a mild cold over having my lungs destroyed any day of the week !

As a US nurse, I received the first dose of Pfizer today. It was a personal relief, but honestly I still worried about how the limited supply and cold storage would limit the ability for everyone to get what I have been lucky enough to receive. Proud of Oxford for this amazing news. Not many realise this was work in progress from 2016. It wasn’t just done in the space of 9 months. The platform had already been layed years ago for anticipation in what they ‘labelled’ Disease X.

Unfortunately, I feel that he is likely too optimistic. The UK authorized the original two full treatments at a four week interval as that was what most of the data was based on. This two full dose program results in 62% to 70% efficacy while the half dose followed by a full dose accidental administration's resulted in 90% efficacy and AstraZeneca says they have figured the dosing regimen to reach 95% efficacy.
So let's look at the numbers, generally the estimate for herd immunity is given by (1-1/Rₜ) but that is if efficacy of the vaccine is 100%. To account for efficacies less than 100%, a crude estimate would be (1-1/Rₜ)/E where E is the efficacy. Now, we've been using a Rₜ of 2.5 for our policies but the post mortem analysis of China's first wave suggest a Rₜ of 5.2 and it has been estimated that about he UK version of concern may add 0.2 to the Rₜ though quite frankly, no one has mentioned what that estimate has been based on. Therefore the likely worse case scenario requires (1-1/5.7)/0.62 of the population to be vaccinated with the Oxford vaccine to reach herd immunity, which works out to be 133% of the population, an impossible target. This doesn't mean the Oxford vaccine won't work as it still seems to keep 100% of the infected who were vaccinated out of the hospital but it does mean that social distancing, hand washing and mask wearing must continue for several months to perhaps over a year even after the mass vaccination program which would be wise regardless of efficacy.
Things are looking up but the vaccines are no magical silver bullet and the authorities have to work on approving the higher efficacy regime for the Oxford AstraZeneca vaccine as soon as possible, preferably before the rollout of the vaccine which is likely within days and of course there is still significant public hesitancy.
I suppose we could specifically ask for the half dose if given the opportunity for the vaccination or more likely wait the 12 weeks before the second dose as Dr. Campbell is alluding to the 95% efficacy being from a greater delay before the second shot not the dosing amount but again I have not been able to confirm this.

I’m in the US AstroZeneca trial in Southern California. They did do a covid test first. I’ve had one shot and the next is Jan 19th. I felt slightly headachy, had a little cough for about a day afterwards. 2 out of 3 trial participants got the actual vaccine so I’m hopefull that I did.

This won't be over for at least a decade, because there will always be the issue of this re-emerging at any point as it spreads throughout the world. You can't vaccinate 90% of the entire population of the world. We may be lucky to get 50% of the world vaccinated and that is simply NOT ENOUGH. When people let their guard down and vaccinations drop in our first world countries to around 30-40% as people return to normality, the virus will jump back and potentially even mutate into something more sinister, where it might even find another vector for infection.

Wonderful news, I hope this means that my vulnerable parents (ages 81 and 90) and nephew with CF, can get the shot here in the US soon. With the Big Three (Pfizer/BioNTech, Moderna/NIH, and soon, the Oxford/Zeneca), supplies should start increasing here soon. So far, my San Francisco Bay Area county of 260,000 population has only received 1,950 doses of the Pfizer, not enough for all healthcare workers and residents of care homes, much less other essential workers and persons at higher risks. I belong to a big HMO and I suppose it will purchase mostly the Oxford vaccine, cost-cutting is the holy grail of the American HMO.

Except he will die vaccine or not and well over half way there. Have people forgotten humans are mortal, it seems so. I'm not taking a vaccination for several reasons and I will also die in due time. Given the data we have on this virus, the risk is of no concern to me personally. If I was in my 70s I would consider it, pretty much knocking on heavens door at that age anyway. Will be good to see if there are adverse effects from mass vaccination over the next two decades

It would of been better to let COVID spread without enforcing any lockdowns ore restrictions. To naturally create group immunity. It's a self resolving solution. We are part of the animal kingdom.. Population control is necissary for al species. These are forces that keep us incheck.. Fighting COVID with vaccines is going against nature. The earth's solution to any species overpopulation is disease. Humans have been pushing the bounderies and limit's of our natural enviourment for to long. Mother earth is old and slow like an ent but eventually she reacts to any disruption of the balance. We should accept that a lot of our sick and elderly are going to have to die. The earlier shorter gap, 21 or 28 days, would have allowed the Phase 3 trials to complete sooner. Adding 60 days to the gap would have added 60 days to the Phase 3 trials and delayed approval 60 days. I think that explains the initial very short gap of 21 or 28 days.

Th UK has one of the best variant detection regimes in the world. This could explain why the variant was detected first in the UK. It may have come from somewhere where the variant monitoring is not so good. Regards to cases of the new variant outside the UK having links to the UK, that could be an artefact of targeted testing. If you believe the UK is the source then you will test UK travellers first. Do they have truly random control groups?
