Something that always helped me is when I'd play Versus mode with someone better than I was. It somehow made me focus more and sync up my steps with them and in a way, their energy seemed to engulf me and push me further. It's hard to explain but playing with someone better definitely worked for me. Also doesn't hurt that I used to play at a Chicago arcade with people like LilQ, WinDEU, KON, and many other amazing players.
One thing that really can mess with your nerves is when the step chart drifts, and the step timing is not right. Sometimes, I'll get a great when my timing is perfect, and it throws me off. Another tricky thing is sudden stops and BPM changes, like in Pluto. The only way to succeed is to just keep practicing over and over. DDR can be a mind game as much as a physical exercise at times.
Could you please list how good you were after playing ddr for 6 months, 12 months, 18 months etc? It would be really interesting to know how quickly you developed and how quickly one realistickly can reach a certain level. So what was the hardest song that you could pass, fc, pfc at each point in time? I think many people would find this really interesting.

Unrelated but still: is there any use playing Stepmania with keyboard to improve DDR reading ability or is it waste of time?
2017-05-14 _ yes there is a lot of use actually, improve mental game.
tbh i first learned reading through keyboard playing, the physical muscle memory came later.
I use stepmania both as a DDR alternative and use the keyboard for muscle memory for hard songs like Cosy Catastrophe CSP
