Wow!! WHAT a compliment!! Thank you so much!
Q no 19 The answer should be choice A.
Basal serum cortisol levels can be used to exclude the diagnosis of adrenal insufficiency and to select patients for additional evaluation with corticotropin (ACTH) stimulation testing. A standard high-dose (250 mcg) corticotropin (ACTH) stimulation test should be performed to establish the diagnosis of adrenal insufficiency in patients with equivocal serum cortisol values in whom hypoadrenalism is suspected.
Godbless you brother! Hoping you reach 50K soon! Is there a pdf version of this?
Hi! Thank you! Yes, at my website
GREAT video. Just a small correction propylthiouracil is the preferred anti-thyroid drug during the first trimester of pregnancy
I know.. was corrected many times on that one.. thank you so much for pointing out. Usually make one mistake per video 😑
BTW Orphan Annie eyes refer to this cartoon character where the eyes are white .. it's from a long time ago. you'll find it if you do a google image search. I think I heard it from the Pathoma videos by Dr. Sattar.
That's hysterical! I created this image using AI - I said - make a fantasy girl with histologic Annie eyes
Thank you so much! Waiting for the resp 😊
Yes! It's on its way!!

You are welcome!!! rrrr... I am so sorry for this super delayed response... =( and yes, pulm is up now
in thyrotoxicosis SVR will increase, that why there will be HTN
it will not decrease as in the video
I don't think that is correct, I believe SVR decreases
isn't propothiouracil supposed to be used in the first trimester because methimazole is not good in 1st trimester (question28)
I noticed that on Q28 you have next to methimazole - mild graves and for use in 1st trimester, however 141 you have avoid methimazole first trimester (the correct answer). Just wanted to give a heads up :)
Thank you... yes, see the comments. That is the 17th time I have been humbled... so important to review all material 100 times!!

They will be on my website, which will be ready in a few weeks
👍👍👍
"Orphan Annie eyes" came from the 20th century Little Orphan Annie comics, which portrayed the character has having unsettling blank eyes with no pupils. Pathologists must've felt the large nuclei in papillary thyroid carcinoma reminded them of their favorite childhood character. Love your work AJ!
thank you thank you thank you
Thank you so much 🙏🏼….waiting for pulmonology next
Question 51, how does vitamin D deficiency cause hypokalemia? Is that supposed to be hypocalcemia and hypophosphatemia?
Whoops!!!
Q72 - SIADH - eUVOLEMIC HYPONATREMIA
23:08 Avoid Methimazole in 1st Trimester
Drugs of Choice
First trimester: propylthiouracil
Second and third trimester: methimazole
I know... so frustrated about that mistake. Been told at least a dozen times.. Serves me right!!
lol no, thanks for the videos. These have been super helpful. I should have just waited because you had it right later in the video!
I believe The pupil nuclei were called orphan annie eyes because the nuclei sometimes resembal the empty pale eyes used in the original comic strips art
Found this channel by chance
Exam in 2 wks , hope this help me
Need you as my teacher for Step 3,, once Step 2 CK gets over !! Love the energy you bring to each video !! CK in 2 weeks and loving these videos for review !!
I will take this request to heart!!! So happy you love the step 2 videos!!
Methimazole is the preferred drug for hyperthyroidism, in the first trimester PTU is the preferred. After the first trimester then you can switch over to methimazole
I know I know I know... I realized my mistake the SECOND after I posted!!! rrrrr!!!!!!!!!!!
Guess u start beta blocker at the same time to control symptom first then u add methinazole
Diabetic kidney disease at first GFR increases
Thats why we give ACE i
To decrease the GFR
And decrease disease progression!
Or so i think….
