  • Short video showing the procedure for a fibreoptic intubation. This procedure is done to insert a breathing tube while you are awake.

    This is the ultimate patient cooperation required for such a procedure.

      i would be freaking out as i suffer from anxiety and i would legit have flashbacks i still have ones from a broken leg from when i was 4 i am 35 now. if had nasal intubation done esp if can't be sedated even concious kind i can't even handle a nasal speclum exam*of course last ent doc i saw well had worst bedside manner and rough) so now i'm even more traumatized even with clonazapem and i am prone to nosebleeds i have had some rather bad ones and whenever i get one i get panic attacks. it would not matter if i was numbed i would still feel and freak out even if eyes closed i cant even wear those nasal canulas they irritate my nose so bad. i do have a deviated septum and on one side looks like i have a nasal polyp. i even refused a nasopharyngeal swab when doctor wanted do one in oct when i was hosptialized for severe case of bronchitis as not only be painful but i know my nose would bleed i have cracks on septum side of nares that easily break. even a nurse or doc saying to be calm it over quickly will not work for me i can't even with my own meds fully control if panic attack i hyperventilate. even if i have flu not have it done. i pretty much know what flu is had few times as child and young adult before testing. and like flu is a virus not much to be done except rest take otc meds drink fluids and lower fever . like who and esp one with anxiety would go to doctor to have swab stuck up in nose and so be like already miserable just to be put through tortue no thanks i'm set sorry be tmi and all. yeah this woman is brave i can handle certain procedures but nothing to do with nose. i had lumbar puncture few years ago handled like a pro i was numbed. i have had procdure for my surgical wound opened up on me on my left shoulder in 2014 from 2 pt sessions i was not even numbed enough and had to deal and i was legit screaming and kept saying hurry 10 mins felt like eternity but i would sooner go through those things than anything stuck in my nose. even my sister who does not have issues like me she is pretty brave and has higher tolerance for pain.and gave birth almost 8 months ago granted she was given epidural but she had to have a c-section but she dealt with very unconfortable attempts at inducing. she had tonsilectomy too and she miserable but she had es manometry done and she said that was pure hell torture and she go through pregnancy and childbirth all over again with her thumbs up.from what i know she has no PTSD from after care of tonsilectomy and manometry but i know she wont have the latter procedure done unless she sedated.

      she probably tough with anything in life lucky kinda makes me wonder though how any one could seem so relaxed for this even with numbing like someone h had to ave smoke a joint right before or could just be me thinking. idk if they didn't sedate me for that now i am no pothead but i would have to be high to go through that along with my anti anxiety med

      It’s adequate for the purposes of the exercise / procedure.

    Having suffered a severe laryngospasm in April of last year because the anesthesiologist didn't take into consideration that I have severe obstructive sleep apnea I don't know exactly what he did after he gave me the midazolam but The laryngospasm ìn the recovery room was so bad I wish they'd have put me back out. Now it's listed in my medical records that because of my SA and other comorbidities that awake intubation be performed.

      If there is Atlanto axial instability we use this method or like oral cancer or any oral pathology like facial dysmorphism which can be hindrance in doing orotracheal intubation

      The woman you see in this video probably would not need and awake fiber optic intubation in reality as she is there for educational purposes. The reason why this is done is for people who are hemodynamically unstable or the risk of not being able to ventilate the patient after anesthetic induction is high (respiratory depression, obstruction etc compounded with a paralytic). Sometimes there are obstructions to the airway or in trauma cases enough blood to where you cannot visualize the vocal cords with a standard laryngoscopy. Over all this woman was a very good sport and if this was genuinely for a procedure I would suspect she had an obstruction or growth of some sort.

    Why would they need to do that? I've been intubated awake in an emergency and that was not fun at all.

      If you have an unstable or obstructed airway. It’s purely for the patients safety as if you put them to sleep before Intubating them they could obstruct so badly and they could die

    yikes is it me or they sticking in the local anethestic thing way into nose i thought it was for the nostrils only. she had neb first i wonder if that numbed her enough for that part

    I had to do this and I can say i imagined it worse. At the end it was okay its not a nice feeling but at least it doesent hurt.

      If a difficult intubation is anticipated this method allows the anaesthetist to safely secure the airway with patient cooperation whilst breathing for themselves spontaneously. Once airway is secured safely general anaesthetic is administered.

    If you're going to put her to sleep then why don't you just do it after she's asleep. Yeah I know makes to much sense.

      because the point of the video it to show that an awake intubation is possible. There are some patients who have difficult airways and it would be dangerous to put them asleep before securing an airway.

      Because the act of 'putting someone to sleep' means that you lose the ability to protect your airway and breathe for yourself. For some patients (who have huge tumours of the mouth, abscesses around the throat, or who have woody necks after radiotherapy), this can mean that it is very difficult to put a breathing tube in as the airway can collapse. By putting the breathing tube in via this technique, you are avoiding a very real risk of death after 'putting them to sleep'

      @@tyur576 A decent anaesthetist should be able to manage a difficult airway with the need for an awake intubation.
      I think it's a lazy option.
      I went through an awake intubation last year
      I needed another surgery this year and different anaesthetist managed to do a normal intubation without difficulty
