I am not aware of a quick reference. My personal go to the UpToDate as they give anticoagulation specific guidance in relation to pre-operative management.
I have a question about the warfarin titration example. If INR is low from diet, drugs, drinking or missed doses and you decide to add that half tab for two nights, do you go back to their regular 5mg/d dose after INR is normal a few days later?
There are multiple factors that must be consider in titrating warfarin. If the low INR is related to drugs or drinking a goal would be to moderate this modifiable factors as opposed to dosing change.
