I’m assuming because he’s on a stage and since it’s assumed the shots were from a pistol he probably was at medium range on ground level meaning the shots would wiz behind him into the air if you know basic trajectory
It looks like the shots came from the left or right side. He was looking to his right, and it went past his right ear, which is what it looks like. He was elevated, so the shots could have gone over peoples heads on the right side.

In the center of the crowd behind the Podium, one frumpy-looking man in green hat and blue blazer, looked right then left, then down pulling the bottom of his jacket, that's when Trump was struck.
