As a firearms enthusiast, there are two possibilities. 1. He had a Glock or similar pistol that doesn't have a manual safety but a trigger safety instead. It's very easy to negligently discharge a trigger safety pistol, Prince why it's encouraged to carry those in a holster rather than in a pocket or in the waistband. 2. The other option is that he's just an idiot who had the safety of his gun off so as soon as he touched the trigger it went off. Either way It's tragically hilarious.
So I don't rock my trigger safety hellcat chambered. But I do keep my external hammer and ambi safety XD chambered, but decocked. I carry both. The nine in the XD buys me time to rack my Hellcat, and I carry spare cat mags.

It's a small enough and cheap enough gun that if I use it in a CCW altercation, I can pick up another easy. It also has multiple safety features to prevent negligent discharges. I never carry with one in the chamber. I've seen too many accidents happen.
I'm not gun expert, but I'd assume a manufacturer like Glock wouldn't leave in such a liability. That sounds like something I'd expect from a Hi-Point.
