Technically, at the end of this story, he just moved away, but it is implied that he followed the same process of Algernon and died as well.
He didn't die. He moved back to the home where he lived before he had the operation.
it is implied he dies, but in the movie it was changed
the ending messed me up bad dude
His landlady is mean.
