Your glue up showed two wedges with the same colours side by side. So an odd number of wedges instead of even. The piece wasn't centered for turning so you got a lot of run out on the outer ring and leaving screw holes in the base??? The details matter. A lot.
I noticed that the piece that was glued up, had a row that didn't alternate woods - which kind of bothered me. But the piece turned on the lathe had all rows alternating. I assume you did a second glue up to correct the issue.
