During the scene in the solitary confinement cell we find out that the Warden (grey dude) tricket Isaiah into thinking he was Lucan, at first Isaiah did not realise but when he did (probably realised after not-Lucan shot another guard) the Warden revealed himself. When he revealed himself Lucan decided to shoot him with the gun. Meanwhile Lucan just walked into a masacre, all the prisoners dead and the guards most likely aswell. He, having walked on everybody shot with no context, hears another gunshot and assumes that the one that shot that gun was also who killed everybody else. So when he goes on his vengence quest to kill who did all THAT, he stumbles upon Isaiah, covered in fresh blood walking out of the cell where the gunshot came from. Lucan assumes that means that Isaiah killed everybody else aswell. Reason for his last lines: "Isaiah? You did this?.. *You betrayed me.*" Which is honestly a really cool parallel seeing as the Warden-disguised-as-lucan also said "You betrayed me." Now for who actually killed everyone, it wasn't stated clearly but I'd bet money on it being the Warden. Seeing as the Warden has already proved to not care about the prisoners health (beating Lucan and the poisened bread), and he also litterally shot a guard.. so... Besides, Isaiah told the other prisoner (the one with the blob fish) that was in the cell when he walked in to run, and said blobfish-prisoner was the only prisoner (that we could see) still alive next to Lucan. sorry about the rant! I got really invested into the lore of this one lol.
