I think that the back singers represent religious aspect. Lacking the health insurance she's relying on God to give her health. Also to wash her hand a few more times just to make sure she's healthy. It's about mental health, how everyone's obsessed with their looks, how she's invisible as an artist unless she's pretty.
In Serbia, health insurance is free. The song is about a superficial understanding of health in the current civilization. There is no spirit or mental health. Obsession with the external appearance and not the inner being of man.

there is a Latin saying men sana in corpore sano "a healthy mind in a healthy body". yet nowadays people worldwide are getting obsessed with physical's health, beauty, celebrities with beauty hair (Megan) and mental health issues are becoming taboo. keep your hair hydrated and rich to look as Megan and not care about inside mental health.
Her style in general is very stream of consciousness, it's the same with her other songs, and it all started when she was reading this shallow, pointless article about the secret behind Meghan Markle's healthy hair and then it all spiraled. It could've been anyone really and it would have the same effect. You have the society's obsession with celebrities and how they dissect every little thing about them, and focus on such details. Then the people's obsession with physical health and the age of self diagnosis and paranoia (the spleen and liver parts). And that's not attractive at all as the artist has to be healthy and by extension beautiful, and the tiny imperfections are what people focus on. Could also be a commentary on the fact that it's some people's way of getting the actual diagnosis, and then how lucky that we have an autonomous nervous system that does its own thing unprompted, that you can enjoy your life without having to think about it, then bam, she remembers that she doesn't have health insurance and how f-ed up is that should something major happen (that's why she surrenders her trust into the autonomous nervous system). Then there's this saying in Serbian - God grant us health, which might be fitting cause the state health system will not cut it, thus she needs to wash her hands and stay healthy. It's also like the healthcare system is washing their hands of the independent artists and people who cannot afford private health care. And mind you, in Serbia you rely on the public hospitals for all the major issues and surgical procedures. Primarily because of the cost, but also not that many private hospitals providing the type of treatment you need. The artist doesn't have health insurance. But wait, how will 'they' follow her and worry in a sardonic way, and know about her and by extension her hair routine, if she's unhealthy and imperfect (all done in the name of health), so she, the artist has to be healthy - though she's invisible to the health care system and past physical facade. The body is healthy, but what about the mind? The mind is unhealthy in the healthy body, the soul is sad in a healthy body, the mind is frightened and desperate in a healthy body, so what do we do now? The absurdity of the lyrics is what carries the commentary. Sounds like a joke cause the whole situation is a joke. The legend says she's still washing her hands - the artist has to be healthy, God grant us health!
