He struck the lottery. I can't get an ophthalmologist on the phone for anything. If I'm lucky, I get an email in 5 days asking "Got a missed call from you; was it important?"
So when I worked for C Spire, I was installing fiber optics and setting up internet for all sorts of customers and government entities. The one time in which I didn't get to install internet at all because I had to wait on the customer was for an ophthalmologist.
@@Rtt473 is that an ophthalmoscope in your pocket or are just glad to see me? vision is WNL is a good way to increase your malpractice courage. you don't need an eye chart , just some quantitative mesure of acuity. like your name tag at 20 feet or your zits at 10 feet. no glasses. try a pin hole and dial in the red or green number on that ophthalmoscope .try charging the handle while you are at it. it might take you almost 10 minutes to get the slit lamp out of storage and then just pretend its a horizontal microscope. Gentamicin drops went out with phrenology. you sent the patient home with topical anesthetic .you are so screwed!!
There’s also an entire Pamdora’s box full of diseases from psychiatry and neurology over rheumatology to oncology and more that may manifest in sudden eye problems (like transient blindness, flashes etc.), sometimes even predominantly as the first symptom that have to be treated as an emergency to preserve vision and/or general health
Seeing Emergency Medicine getting slowly annoyed even though they called the consult was such a breath of fresh air, I thought only Neurology was able to achieve that feat 🤣
I had a ED doc get mad at me after he called me for a consult. I was telling him what im ordering on the phone (while ordering it in Epic) and he thought i was asking him to order it. When he realized i was in there hes all mad at me "if you're going to order it why you tell me then" :::facepalm:::
Reminds me of an ER doc that was playing bejeweled the whole time I was giving report, so I made a snarky comment at the end to get his attention. He looked up and repeated everything back to me and gave me his order. I gave him and impressive nod and said “Will do.” He then put on his readers in a cool guy fashion and slowly turned back around in his chair to dictate. I had plenty of good times/interactions with that doc after that.
During a ward round whilst I was a resident, the prof stayed behind the rest of us by a few metres, looked disinterested and was just chit chatting with the senior nurses; then the most senior Professor and Chief of the division joined the round while we were on the ninth patient. The tradition in the division is that when a more senior consultant joined the round, the most senior person leading at that point who was present from the beginning gives him a summary of the patients that were seen before his arrival. The Prof that was chatting with the nurses being the senior gave the summary with out missing a beat or any important detail about any of the patients. I learnt never to mess up with him from that point onwards and also had to up my game.
Wow. So Saturday morning on my 28th birthday my chicken pecked me in the eye. I went to the tiny local ER and I’m pretty sure I was the only patient there. Once they had me back in a room, I waited three hours before the ophthalmologist came and saw me. Yes, HE CAME AND SAW ME. At the time I was annoyed it took him so long, but now I realize I received the greatest honor of my life. Who knows what the on call doc had to do to get him in. I witnessed a miracle. I am humbled.

I’m a retired family doc. One time about 10 years ago I had a patient who I thought was having a posterior vitreous detachment. She was the last patient of the day at about 5:45 pm on a Friday. I wanted to at least touch bases with the ophthalmologist on call both for the patient’s (and my) peace of mind. I called his office and the office was understandably closed so I left a message with his answering service for him to give me a call at my office where I was still seeing patients. Thirty minutes went by- no return call. I called again. Meanwhile the patient was waiting in the exam room and I asked my medical assistant to stay late as I never had a female in the office without another female employee present and in sight. another thirty minutes went by- no return call. I called a third time and told the answering service that I and my medical assistant were going to drive the patient in my own car to his home and we would be waiting in the driveway for him. I received a call back within 5 minutes. He was at his son’s Little League game. (I was never able to get to those.) Anyway, after a discussion and reassurance he agreed to see the patient at his office first thing Monday morning. Everything was duly noted in my chart and I told the patient to take it easy over the weekend and I gave her my cell number to call if her vision worsened.
Sounds like all of your patients were lucky to have you, you really went above and beyond. My childhood family doc dismissed all of my health problems, but when I switched to a new one a few years ago I really lucked out. I have so many health issues to work through because of the years of neglect, so he schedules a call every month on Mondays because that's the day where he does paperwork from home and knows that it'll be fine even if the call goes over an hour, because it means we can address everything and get a bunch done in one appointment! I often end up finishing the appointment with a change to meds/dosage and at least one referral or requisition. Thank you for actually caring for your patients. The world needs more family doctors like you. More family doctors in general, tbh, y'all are so overworked especially in countries like mine with public healthcare which doesn't pay nearly as well as private
@@BruceGinkel they aren't necessarily an only parent, and I bet this hypothetical son would be far more proud and grateful to have a parent who has doubtlessly saves thousands of life than if they showed up to sports games which don't matter that much in the grand scheme of things. Doctors can still find ways to show their family that they love and appreciate them, even if it's a profession with little free time to spend with family. Actually my family doc is a dad and I'm sure he's doing just fine because I know they get vacations, go sledding, camping, hiking, and spend plenty of time together. Because his family came over from another country with different weather patterns (UK vs north america with hotter summers and colder winters) I sometimes will give suggestions or ask how they're doing with the weather, so I've heard some vague details and know enough to say that, though he maintains a professional distance and I don't pry for details 😊 his family and patients are lucky!
This channel continues to prove that my time seeing an ophthalmologist at insane o’ clock in the morning in the ER was such a miracle. It also explains why the doc was grumpy.
Reminds me of my accountant. The man was sleeping during a seminar. Lecturer asked him to repeat the last segment. Perfectly recapped the last 10min and answered the follow up question. Closed his eyes immediately when the lecturer went on. Guy was not human sometimes

This is the best one ever. I'm an ED doctor and this is so accurate!!! My exact reaction when a specialist answers the phone, who actually asks me details... and the long "made-up words" 🤣 And yes, going back to play instead of trying to do a test that I never even heard of 🤣🤣🤣
Once again, I'm immeasurably thankful to our hospital for keeping an ophthalmology resident perpetually locked in a tiny dark room in the ER as part of the "learning process". I can just shove the kid in through the door and go back to doing my job.
I got the neurologist in at 5 am once, had to meet him in the carpark and physically take him to the patient. He then fell asleep whilst examining the patient, we didn’t know if it was kinder to wake him, or let him nap for a bit.
I will forever be grateful for the ophth who agreed to come in right away as they were scheduling me for immediate brain surgery to address IIH. He was a sweet older man who told me during the exam that he had bought tix for a rare movie date with his wife, but that it was okay to reschedule his day. He let neuro know that it was okay to address my dwindling vision with medication and see how I responded before putting a shunt in my brain (a surgery that neuro bullied me into and didn't fully explain before scheduling). Several years later I'm still vision impaired, but not blind as neuro insisted I would be, and after reading up on the surgery after discharge, I'm so thankful I didn't have it done. Ophthalmologists are heroes!
Thank goodness the one time we needed Opthalmology they came! Of course my daughter's eye was suddenly bulging out her head so technically everybody came that night! But still... I'm very grateful because after my daughter was admitted Opthalmology consulted with colleagues across the country and figured out that it was pressure building up that was literally dislodging my daughter's eyes. That's Jonathan type dedication and service! (Her eyesight was saved by a quick spinal tap to relieve the pressure while blood thinning medication broke up the blood clot causing the pressure!)
Oh my goodness that must have been so scary for her!! I'm so glad you had a team around that cared and you were able to get everything sorted out. I hope your daughter is doing better now! 💜💜💜

That actually sounds like Chinese Medicine. Liver often overreacts or invades Spleen. It looks like digestive issues resulting from stress. I started rattling off a bunch of acupuncture points in my head, then nodded when he mentioned Kidneys. Of course! Water is Mother of Wood! Dont mind me, I'll just go back to my corner now.

We diagnose by interview, so a lot of symptoms can get treated at once. I have never seen a person leave a health care office as peaceful as after acupuncture. I call it acuphoria.
Legit had a patient this weekend with a right eye infection. Ophthalmologist told me it was necrotic, perforated and to assess for "eye contents" that would potentially "shoot out". Plan was surgery on Monday. I have never been so cautious with a patient in my life... the 2 things that I will never be comfortable working with as a nurse are eyes and phlegm. As someone who knows nothing about medical things, I am inclined to believe all these sketches as truth, especially when I read the comments and everyone is laughing at things I don't understand or going "yeah, I feel that"
The general idea that the sketches are based on are very much accurate. Eye stuff is foreign to almost every other doctor. The sketch makes ER maybe seem a bit lazy, but they also have a massive volume of patients and wouldn't get through them if they didn't turf things out and move on to the next patient. The anesthesia stuff also. The sudoku isn't real, not myself or any of my anesthesia partners do sudoku, but we all either read a book, read the news, play chess, or do something to keep our minds somewhat alert when there's nothing going on in a long surgery (staring at the vital signs screen is actually rather hypnotic).
I woke my daughter up laughing so hard- spleen fighting the liver had me in full blown asthma attack as I was already cracking up from about 15 seconds in…. So rough morning tomorrow but worth it. Jonathan, please show the good doctor this comment. Also, I need strabismus surgery due to exotropia diplopia- and the surgery has me so freaked out I seriously only want Dr Glaucofleken 👁 ❤️
My uncle recently had some flashes in his vision and was scared that his retinal tear (last operated on in 2008) was tearing again. Immediately called the Opthalmology's office (I was excited that I knew it would be Opthalmology thanks to these skits), and waited for them to call back with instructions while he rested. An hour later we heard back from them, and the doctor had the opinion that it was just stress on the eye and to come see him bright and early Monday morning (this was a Sunday evening) just to be sure. Sure enough, it wasn't tearing and it was fine. I just thought it was humourous because it definitely matched these skits' theme of Opthalmology not working outside of regular hours 😂
Ahhh, it's so true. I had to go to the ER. Phone consult with the ER doc was spot-on. The opthalmologist was truly patient and concerned for my well-being... Then saw me in clinic in the morning. 👀

As an aspiring ER doc, I plan to have a Opthalmology cheat sheet list before paging. It'll write, "Say these words to get them seen Not tomorrow' (i) central retinal artery occlusion/CRAO or (ii) globe rupture or (iii) syphilis conjunctivitis. Make sure to add the word I THINK it's ______ and then remember to nod at Johnathan.
I tried the globe rupture once telling her that the patient's pupil was misshapen,but she(an ophthalmologist) just yelled at me for calling her at 3 A.M,and told me to send the patient to the eye clinic in the morning then cut the call
Uk Optometrist here, had a patient who was seen by A&E (emergency department) for flashes and floaters, told nothing was wrong. He came in a day later to see me as he was concerned, took one look with my volk lens and saw his retina flapping around! (he's fine now, referred directly to ophthalmology). I think people don't see opthalmology as very important but most people would rather lose an arm than go blind.
An Optometrist/Orthopist/Ophthalmology detected, analysed, and fixed my strabismus and gave back my 3d vision. Maybe not an arm, but being able to see as others see is wonderful. And knowing that the surgeon had done 4000 of these procedures was awesome
When I was a kid I had to have an emergency visit to the Ophthalmologist to have little spikes from a chestnut husk removed from my cornea. (don't look up at a chestnut tree during a wind storm!) I saw him the morning after it happened.
This is brilliant. I love the fact that it translates perfectly to hospital culture in the UK (including the cycle helmet) and, I suspect, just about anywhere in the developed world!
Some of his stuff is US specific (insurance battles, etc.), but a lot of the issues regarding doctor/doctor interaction and patients seem universal -- in many cases even in the developing world.
Used to work with a a dude who would ask my opinion, then argue no matter what. He was going to do what he wanted, but was used to arguing about it, and felt like it was wrong if I someone agreed with him.
Amazing as an ER doc that very accurate. I would say we would have done our Slit lamp exam as well as a Bedside ocular Ultrasound. And call so that they can see him in the AM. Brilliant as always. The ER doc would actually tell the Optha to come in, chart the "informed time" and set up a ticking clock to count down the clock when he gets to make a "Forced admission" from the ER when it rings.

Fun fact: Sauté is often used by both the English and the French to mean pan frying food, but professional french cooks will likely wince at this definition knowing because sauté is a different cooking method all together.
Jonathan, if you're really looking to imbue your dish with lots of garlic flavor, I would throw the garlic in towards the end instead of sauteing it. The reason is that sometimes sauteing it can make it burn/taste bitter Sauteing onions in advance might be a better option.
ah it depends on what kind of garlic flavor you want. If you want the sharp pungent raw flavor, mince the garlic finely, sprinkle it with kosher salt and use the flat of the knife to grind the garlic and salt on the cutting board into a fine paste. Scoop it up and leave it in a small dish for the enzymes to develop the full flavor for 10 minutes. add at any stage of cooking. If you want a mellow nutty garlic flavor... Trim the tops off the bulb of garlic, about ½ a cm. drizzle with good Olive oil and balsamic vinegar, wrap in heavy foil then bake at 350f till soft. squeeze out of skins after it cools, mash and add to any dish as needed... or eat it with a spoon. Or smash/mince/slice/boil/saute/Deep fry/Shallow fry/eat raw/Ailoi/BURN your garlic as you prefer it! It is all about your preferences Jonathan... um the Ophthalmologist preferences... do you even eat Jonathan? How do you know what the food tastes like before you serve it?

As a third year internal medicine resident in the ED one night around 2 am, we were on our 12th admission of the night, tag teaming the exam with the intern. Because it was an elderly alcoholic with pancreatitis, we kept the patient steady by placing our thumb and index finger on the brow and cheek while using the ophthalmoscope. Everything was in focus and suddenly it was not, I looked down and saw the eye hanging out of the socket. It slowly sucked back in. On the phone immediately to the ophtho resident. Of course he did not come in, just said the muscles get lax when they are old and it will be fine since it went back in place. And he went back to sleep. I can still get the creeps from the image 30 years later.
I just recently had a vitreous detachment. And the major floater that I now have is driving me nuts.... but at least I actually kind of understand this video in a way I would not have a few weeks ago.
I always have floaters. Way back in the late 60s, I got a Superman floater. I used to “fly” him back and forth across my visual field. He finally disintegrated about 20 years ago.

I was once sent from Emergency to an adjacent medical building to see an Ophthalmologist and when I was examined she said my eyes were in spasm; I was having my first ever migraine. No Jonathan was seen in the clinic.

I have posterior vitreous detachment. The floaters are really annoying. Now I have to pay a retinologist every 6 months for him to tell me "yup, everything's fine." At least his retina scan shows exactly what I see and proves that it's not just regular floaters and I'm not crazy. I overhear the other patients in the waiting room and he gives them pretty much the exact same spiel as me. Turns out most retinologist's patients are PVD. So based on what my doctor tells me to watch out for, flashes + floaters could be retinal detachment and is very bad.
Fun fact, I’ve never seen an ophthalmologist as an imaging tech in 7 years working. I don’t know if they even exist in hospital walls or anywhere for that matter, literal unicorns!

I have so many health issues to work through because of the years of neglect, so he schedules a call every month on Mondays because that's the day where he does paperwork from home and knows that it'll be fine even if the call goes over an hour, because it means we can address everything and get a bunch done in one appointment! I often end up finishing the appointment with a change to meds/dosage and at least one referral or requisition.
Thank you for actually caring for your patients. The world needs more family doctors like you. More family doctors in general, tbh, y'all are so overworked especially in countries like mine with public healthcare which doesn't pay nearly as well as private

Doctors can still find ways to show their family that they love and appreciate them, even if it's a profession with little free time to spend with family.
Actually my family doc is a dad and I'm sure he's doing just fine because I know they get vacations, go sledding, camping, hiking, and spend plenty of time together. Because his family came over from another country with different weather patterns (UK vs north america with hotter summers and colder winters) I sometimes will give suggestions or ask how they're doing with the weather, so I've heard some vague details and know enough to say that, though he maintains a professional distance and I don't pry for details 😊 his family and patients are lucky!
This channel continues to prove that my time seeing an ophthalmologist at insane o’ clock in the morning in the ER was such a miracle. It also explains why the doc was grumpy.

Once again, I'm immeasurably thankful to our hospital for keeping an ophthalmology resident perpetually locked in a tiny dark room in the ER as part of the "learning process".

I got the neurologist in at 5 am once, had to meet him in the carpark and physically take him to the patient. He then fell asleep whilst examining the patient, we didn’t know if it was kinder to wake him, or let him nap for a bit.
I will forever be grateful for the ophth who agreed to come in right away as they were scheduling me for immediate brain surgery to address IIH. He was a sweet older man who told me during the exam that he had bought tix for a rare movie date with his wife, but that it was okay to reschedule his day. He let neuro know that it was okay to address my dwindling vision with medication and see how I responded before putting a shunt in my brain (a surgery that neuro bullied me into and didn't fully explain before scheduling). Several years later I'm still vision impaired, but not blind as neuro insisted I would be, and after reading up on the surgery after discharge, I'm so thankful I didn't have it done. Ophthalmologists are heroes!

Thank goodness the one time we needed Opthalmology they came! Of course my daughter's eye was suddenly bulging out her head so technically everybody came that night! But still... I'm very grateful because after my daughter was admitted Opthalmology consulted with colleagues across the country and figured out that it was pressure building up that was literally dislodging my daughter's eyes. That's Jonathan type dedication and service!
(Her eyesight was saved by a quick spinal tap to relieve the pressure while blood thinning medication broke up the blood clot causing the pressure!)

That actually sounds like Chinese Medicine. Liver often overreacts or invades Spleen. It looks like digestive issues resulting from stress. @@lambentlamprey Thank you. I find it fascinating to view the body from a holistic perspective. It is like geometry instead of algebra. We diagnose by interview, so a lot of symptoms can get treated at once. I have never seen a person leave a health care office as peaceful as after acupuncture. I call it acuphoria.
Legit had a patient this weekend with a right eye infection. Ophthalmologist told me it was necrotic, perforated and to assess for "eye contents" that would potentially "shoot out". Plan was surgery on Monday. I have never been so cautious with a patient in my life... the 2 things that I will never be comfortable working with as a nurse are eyes and phlegm. Keep up the good work Ophtho....
As someone who knows nothing about medical things, I am inclined to believe all these sketches as truth, especially when I read the comments and everyone is laughing at things I don't understand or going "yeah, I feel that"
While over the top of course for humor, the vast majority is accurate at the core.
I work tangentially to medicine, everything I've learned so far about medicine matches up with these sketches.
I work with multiple specialists in a hospital. It's pretty darn accurate! 😆😕
The general idea that the sketches are based on are very much accurate. Eye stuff is foreign to almost every other doctor.
The sketch makes ER maybe seem a bit lazy, but they also have a massive volume of patients and wouldn't get through them if they didn't turf things out and move on to the next patient.
The anesthesia stuff also. The sudoku isn't real, not myself or any of my anesthesia partners do sudoku, but we all either read a book, read the news, play chess, or do something to keep our minds somewhat alert when there's nothing going on in a long surgery (staring at the vital signs screen is actually rather hypnotic).

I woke my daughter up laughing so hard- spleen fighting the liver had me in full blown asthma attack as I was already cracking up from about 15 seconds in…. So rough morning tomorrow but worth it. Jonathan, please show the good doctor this comment. Also, I need strabismus surgery due to exotropia diplopia- and the surgery has me so freaked out I seriously only want Dr Glaucofleken 👁 ❤️

My uncle recently had some flashes in his vision and was scared that his retinal tear (last operated on in 2008) was tearing again. Immediately called the Opthalmology's office (I was excited that I knew it would be Opthalmology thanks to these skits), and waited for them to call back with instructions while he rested. An hour later we heard back from them, and the doctor had the opinion that it was just stress on the eye and to come see him bright and early Monday morning (this was a Sunday evening) just to be sure. Sure enough, it wasn't tearing and it was fine. I just thought it was humourous because it definitely matched these skits' theme of Opthalmology not working outside of regular hours 😂

As an aspiring ER doc, I plan to have a Opthalmology cheat sheet list before paging. It'll write, "Say these words to get them seen Not tomorrow' (i) central retinal artery occlusion/CRAO or (ii) globe rupture or (iii) syphilis conjunctivitis. Make sure to add the word I THINK it's ______ and then remember to nod at Johnathan.
The words 'globe rupture' strikes fear in all OR staff! ALWAYS an emergency and never gonna be a good outcome

Uk Optometrist here, had a patient who was seen by A&E (emergency department) for flashes and floaters, told nothing was wrong. He came in a day later to see me as he was concerned, took one look with my volk lens and saw his retina flapping around! (he's fine now, referred directly to ophthalmology). I think people don't see opthalmology as very important but most people would rather lose an arm than go blind.
An Optometrist/Orthopist/Ophthalmology
detected, analysed, and fixed my strabismus and gave back my 3d vision. Maybe not an arm, but being able to see as others see is wonderful.
And knowing that the surgeon had done 4000 of these procedures was awesome

This is brilliant. I love the fact that it translates perfectly to hospital culture in the UK (including the cycle helmet) and, I suspect, just about anywhere in the developed world!
Some of his stuff is US specific (insurance battles, etc.), but a lot of the issues regarding doctor/doctor interaction and patients seem universal -- in many cases even in the developing world.
Used to work with a a dude who would ask my opinion, then argue no matter what.
He was going to do what he wanted, but was used to arguing about it, and felt like it was wrong if I someone agreed with him.
Amazing as an ER doc that very accurate. I would say we would have done our Slit lamp exam as well as a Bedside ocular Ultrasound. And call so that they can see him in the AM. Brilliant as always. The ER doc would actually tell the Optha to come in, chart the "informed time" and set up a ticking clock to count down the clock when he gets to make a "Forced admission" from the ER when it rings.

In french 'é' is the past tense suffix equivalent of 'ed' like in 'moved' or 'climbed'.
Fun fact: Sauté is often used by both the English and the French to mean pan frying food, but professional french cooks will likely wince at this definition knowing because sauté is a different cooking method all together.
Jonathan, if you're really looking to imbue your dish with lots of garlic flavor, I would throw the garlic in towards the end instead of sauteing it. The reason is that sometimes sauteing it can make it burn/taste bitter Sauteing onions in advance might be a better option.
ah it depends on what kind of garlic flavor you want. If you want the sharp pungent raw flavor, mince the garlic finely, sprinkle it with kosher salt and use the flat of the knife to grind the garlic and salt on the cutting board into a fine paste. Scoop it up and leave it in a small dish for the enzymes to develop the full flavor for 10 minutes. add at any stage of cooking.
If you want a mellow nutty garlic flavor... Trim the tops off the bulb of garlic, about ½ a cm. drizzle with good Olive oil and balsamic vinegar, wrap in heavy foil then bake at 350f till soft. squeeze out of skins after it cools, mash and add to any dish as needed... or eat it with a spoon.
Or smash/mince/slice/boil/saute/Deep fry/Shallow fry/eat raw/Ailoi/BURN your garlic as you prefer it! It is all about your preferences Jonathan... um the Ophthalmologist preferences... do you even eat Jonathan? How do you know what the food tastes like before you serve it?

As an intern, once saw 3 different Ophtha emergencies in one night, the Ophthalmologist was so pissed to get that many calls
As a third year internal medicine resident in the ED one night around 2 am, we were on our 12th admission of the night, tag teaming the exam with the intern. Because it was an elderly alcoholic with pancreatitis, we kept the patient steady by placing our thumb and index finger on the brow and cheek while using the ophthalmoscope. Everything was in focus and suddenly it was not, I looked down and saw the eye hanging out of the socket. It slowly sucked back in. On the phone immediately to the ophtho resident. Of course he did not come in, just said the muscles get lax when they are old and it will be fine since it went back in place. And he went back to sleep. I can still get the creeps from the image 30 years later.

I always have floaters. Way back in the late 60s, I got a Superman floater. I used to “fly” him back and forth across my visual field. He finally disintegrated about 20 years ago.

I have posterior vitreous detachment. The floaters are really annoying. Now I have to pay a retinologist every 6 months for him to tell me "yup, everything's fine." At least his retina scan shows exactly what I see and proves that it's not just regular floaters and I'm not crazy. I overhear the other patients in the waiting room and he gives them pretty much the exact same spiel as me. Turns out most retinologist's patients are PVD. So based on what my doctor tells me to watch out for, flashes + floaters could be retinal detachment and is very bad.
