Most of Pablo's songs resonates to us in one way or another. It's like he has written them specifically for you so I agree with what you said that this song feels like he knows you and have written this song for you. He's a modern poet. He qrites songs that are very relatable. Unconciously he has touched our hearts with his masterpieces. Being artist of the year really fits him, for any year even.
With Pablo's songs it's always deep and you cannot grasp the message right away because it's art ,his metaphors it's not one of those bubblegum music or just girlfriend boyfriend heartbreak. You could watch it from last year's mall shows.

I think in the album they made it aofter as it deals with depression or hardships. The original take is similar to what he did in the US tour. My first time to hear and I like it instantly the song and the delivery. I went back and watch him sing it live forat time I guess the emotion got the better of him. Past experiences may still be fresh, he got overwhelmed by the support of the fans and thank them, while breaking down, out of gratitude.
Pablo is a great storyteller thru his songs. He leads the listeners into vulnerability and reality. He's so relatable. But he does not end there. He always gives hope and expresses gratitude for what he learned through these hardship and experiences. He's truly an admirable artist and an individual.
