problem with your "perfect charting" is that it would take 4 hours to complete for each patient(not including phone calls...)
or 6 hours AND THEN no one reads or appreciates it...98% correct one error and you are stupid. Get rid of these 200000$ a year freaks who never see a patient or work in the REAL world
