    Phenomenal video. I learned a lot to help myself with this chronic problem I have almost exactly what she has. Only problem is I have no one to work on me professionally or personally, so it's all on my own regarding home-based remedies. I will keep studying your video based advice.

    Great video. I was just diagnose of having L4-L5 diffuse disc bulge, L5-S1 posterior central disc bulge. I was recommended to do PT but was not really doing well with my therapist so I just do yoga for lower back pain. I feel hopeless but this video gives me an insight how to lessen the pain in my left ankle. My Doctor doesn’t recommend me to do chiropractic, only a routine stretching.

    Hi!! these are standard standards for painful herniated discs. I don't have pain anymore and my L5 DISC has actually burst, it is now very thin. I do these exercises and others at home every day :I don't have pain now, thee are great and standard exercises. However, on the internet I never find video about exercises that will help you improve your lower back strength: something specific for people who have bulging or broken discs. As far as I know one has to improve the muscles to protect those vertebrae that don't have anymore a functioning disc. The exercises you are showing here are good in the first phase (feeling less pain), I WOULD LOVE TO SEE SOME strength exercises specific for lumbar herniated discs. Thank you!

      I did a few of these exercises in the past (since I was doing lots of core exercises and pilates) in spite of that I still destroyed a disc... probably I ALREADY HAD A BULGED DISC , I WAS NOT AWARE OF IT AND I engaged in other more dangerous exercises : I have always struggle with any exercises involving bending forward for example. I will need someone to guide me back into doing them correctly and step by step...I am really scared of attempting doing any exercise by myself. Anyway, great video, thank you very much, it looks great.

    All what is described she has is exactly to a tee what I'm experiencing even the same leg. Will keep trying these exercises and if there is another video on home exercises as well please send a link. 🙏 Am curious though, stupid question how the bulging would work if you have spinal bifida occulta L5. Isn't it a space there?

    The symptoms you explained first in the video is exactly what I've been struggling with. I was "diagnosed" by a chiropractor, he told me it was piriformis syndrome. Which has led me to mostly doing deep tissue massage(no stretches has been helping). I'm gonna give these rotational exercises a go. However, I do not get any pain from bending over, like she does, I get it when leaning backwards - what difference does that make? Also, when I do the Mackenzie I get a sharp pain in my lower left buttocks.

      +Rune Posselt The patient in the video receives pain from bending over because of her pathology: a postero-lateral disc bulg that is in impinging on the nerve roots. Mckenzie has 2 schools of thought: flexion preference and extension preference. He demonstrated extension preference here that is common in treating disc pathologies as he explained. If he have pain going into extension (backwards) then it is more likely something like foramenal compression that is impinging on the nerve roots and Mckenzie flexion exercises would be beneficial. That said, take this with a grain of salt as I haven't seen everything else you present with :)

    I understand the rationale for flossing but is it safe to bring hip into flexion this much? Would this not increase lumbar flexion and lead to increased hernia? I really liked the traction technique and think it'd be good to use with McKenzie's techniques but unsure about flossing in this particular case! All the best

      Only move into flexion of the hip that you can keep neutral spine. Some people have more hip ROM than others

      Normally yes, you move towards the painful side. 1min x 3 sets, and 3-4 times per day in the acute phase

    I see why you did the McKenzie extension, left side bending.
    But left rotation ?? Why ?? Mechanically and anatomically I'd say do a right rotation.
    And the patient reported relive with left rotation !! Any explanation?

      left rotation 'closes' the disc down on the left sided where the bulge is. This is following McKenzie principles on disc material movement and basic physics.

    I have exactly the same problem . Not had physiotherapy yet. Sick of waiting for NHS appointment(3 months). Do you think an invertion table will help .. i have l4l5 disc bulge pressing on l5 bialateral nerve.

    So if rotation of the disc, re-centralizes it, what was the cause of the opposite rotation to disc in the first place? Isn’t that what you should be treating?

      @@physiorehab Thanks for replying! The treatment looks very cool but what caused the rotation in the first place? How do you treat that? Remedial exercise is conscious and new movement patterns must be repeated thousands of times to create permanent neuroplastic changes. What about eyes, feet and crawling patterns or retained primitive reflexes that limit conscious effort of remedial exercises? I think we rely too much on actual physical treatment.

    I was told to do the bird dog exercise. Should you wait until your completely pain free before doing it? I can't really bend forward to well right now so when should you start with strengthening

    Doctor I started having tingling sensation in my foot which developed to severe thigh pain. Mri finding showed mild stenosis in neck and disc bulge at l3 l4. Doing physio exercises for that. But main concern is erectile dysfunction, which my doctor is not accepting I'm 27 yr old(no anxiety or stress). Can i still recover by physiotherapy or need surgery. Is this reversible

    So I have symptoms bilaterally. Is that a central bulge or multilevel bulge? I tried traction from hanging and rotations but that seems to aggravate. I assume just stay w/the extension?

      he probably meant loaded and/or fast twisting actions. The lumbar spine by itself does not allow a huge amount of rotation. However if the person has proper spinal segmentation in the whole spine it should be a big problem to get into this position (slow and no significant torque), the movement and forces will dissipate nicely along the involved structures. In this case he probably just wants to help centralize the disk material and the pain. You can see how careful he was as he helped her get into position. Off course, I don't know your exact condition.
