Mike Starr wasn't trying to hide a laugh. He was genuinely pissed off by the most annoying sound in the world and him screaming at the top of his lungs was actually directed towards Jim.
Norm McDonald broke character in Dirty Work when Don Rickles was talking to Artie Lange’s stomach in the movie theatre scene. Norm couldn’t keep it together so they finally decided to keep his laughing in.
It's important to remember that when an actor improvises a line that makes the other characters laugh, in nearly all cases, they have to shoot the other characters' laughter in a follow-up shot, setting up the camera to shoot in the opposite direction, changing lighting, etc. -- so the reaction to ad-libbed lines is usually not happening at the same time as the ad-lib (except in the case of a 2-shot or wide shot).
