Half of the people I know in and out of my family go by their middle names. Not until recently have I ever heard of anyone thinking that there is an issue with that. It's done for many different reasons but it's still their birth name. I find it extremely strange that people have a problem with that.
I worked for a Doctor who allowed the staff to call him by his first name until I came in as office manager. The title was earned with years of schooling!

Growing up it was grandma and grandpa. I try to keep it like that. It drives me nuts too. My dad was named Crazy Dude by my nieces and nephews. lol
