Hi Yoke Moi, print out the form template and let your oncologist fill up will do. The template link is at the description of the video.

在新加坡治疗我不清楚哦，你可以打电话去询问。你现在没有在马来西亚每年复诊吗？如果有的话，让你的专科医生填表格申请就可以了。
