  • The temporary anchorage devices constitute essential tools for the present and future of Orthodontics. However, the use of TADs does not exclude the need of a solid biomechanical foundation for its appropriate applications.
    Correction: 03:03 TIP: Put a stop hook or ômega loop flush to molar tube - this will increase the efficiency of this mechanics!

    Doctor, two questions:
    1) in clinical situations, if there no risk of complication when the second premolar contacts the miniscrew during distalization procedure?
    2) if the miniscrew (and line of force) are below the purported center of resistance (low buccal frenum etc.) of upper arch, incisal plane canting may occur. What can be done to reduce this side effect?

      1) there is risk if TAD is closer to 2nd premolar rather than 1st molar. That's why IZC miniscrew is better placement
      2) canting of incisal plane would occur if miniscrew on other side is on different hight. When you change the hight of miniscrew or hook your rather change overbite during ditalization

    Screw will impair the movement ,so infrazygomatic screw is advisable

      Yes Dr! The video was made before the development of extraoral screws. Today they are good alternatives for upper arch distalizations.

    One question will we add force required for every tooths bodily movement for distalization in this case.?
    Or 200mg would be enough?

    • @essentialbiomechanics9381
      @essentialbiomechanics9381  3 หลายเดือนก่อน

      200 g is enough for retraction of the anterior group of teeth

    • @haris5665
      @haris5665 3 หลายเดือนก่อน +1

      Aren't we retracting the whole arch as we are applying force on the wire. . How much force would be enough for the arch Sir. 200?

    • @essentialbiomechanics9381
      @essentialbiomechanics9381  3 หลายเดือนก่อน

      Sorry, now I got your question. The answer is the same - yes, 200 g is enough even for the entire distalization of upper arch. It may seems strange that we can use this same magnitude for different purposes, but it's true. Literature has shown that around 200gm is enough even for distalization of the entire amndibular dentition - Roberts WE, Viecilli RF, Chang C, Katona TR, Paydar NH. Biology of biomechanics: Finite element analysis of a statically determinate system to rotate the occlusal plane for correction of a skeletal Class III open-bite malocclusion. If the 3rd molar is erupted or impacted, Should we extract it in this case before attempting for destalization?

    • @essentialbiomechanics9381
      @essentialbiomechanics9381  2 ปีที่แล้ว +2

      If the 3rd molar is erupted (an out of occlusion, of course), It's better to extract it. If it's impacted, some surgeons prefer to keep it impacted. Depending on its position, it will never cause any problem.

    • @essentialbiomechanics9381

      Only if a great amount of movement is needed. In the present case, the Class II is so mild that this will not be a problem.

    Just a suggestion: it would be easier to tie the spring on the hook first and on the tad after.

      It depends on the degree of the Class II. Normally, the movement rate is 0.5 to 1mm/ month.

    Se puede hacer esa retracción asi defrente sobre el miniimplante? No se tendría q primero distalizar esa primera y segunda molar y luego ya retraer el sector anterior? Alguien colega ortodoncista q opine al respecto con base xfavor.
