I mean, end portal frames have facing, so for that "left or right" one it depends on how the portal frames were placed. All the frames have to be facing the center for the portal to activate. Also, the long dripstone stalactite only falls one block before it hits the villager, then it breaks into all the pointed dripstone items that it's made of. That villager only took as much damage as if it jumped onto a piece of pointed dripstone placed on the ground.
