It's because of a stupid bug that highlights your UI as if you pressed an ability and so you instinctively press r to get out of it, but it just uses your ult
Yeah like what the other person said, you can see before pressing R Hwei is stuck in the pose like he is ready to cast an E spell, but it's already on cooldown because he just cast EE. I assume the bug happens by spamming the buttons too quickly, and Hwei manages to enter a subject again at the same time it is cast.
