We had an issue similar to this on the Pinfari 64 m coaster we had at the park I worked at. The front upstop roller on the left front truck set of the car came up on top of the rail in the last curve before the station. In our situation though the ride was shut down until we could figure the problem and have it corrected. What we found was that the outside rail on the last curve had egged over time as was not round in that section any longer. We replaced the rail in accordance with Pinfari's instructions (also all track was meticulously checked to see if any other spots were egged). Pinfari sent out notices for all operators to check for the wear like we experienced. Our coaster ran at that park with no other incidents for another 7 years until the park closed and all rides were auctioned off. It is still running. It was sold to Celebration City then a few other parks if I remember correctly. Last I heard it was at an FEC and was called the Big O but I think it has been relocated since then.

I'm glad that nobody was killed in the crash. I do not understand how a park could be so careless as to let a coaster do what the manufacturer said it should never be allowed to do. Those trim brakes are there for a reason, and the park should not have disabled them at all.
Also, I admire your honesty about a mistake you made years ago, in a video that most people probably don't remember the contents of (I forgot that it even existed, though the coaster did seem familiar for some reason).

If the park was in financial trouble, as according to the video, they probably didn't have the financial wherewithal to properly modify or repair the ride properly to begin with. The shortcuts, ignoring of the problems, and covering up other problems was a desperation move to keep a potential moneymaking ride active when it should have been shut down and repaired properly.

This coaster operated at Del Grosso Amusement park (formerly Blands Park) from 1987 to 2003 under the name "Zyklon." So it was at that location for 16 years. It was my favorite ride at that park and rode it more times than I can count as I lived only 30 minutes from it. This is the coaster that sparked my love of roller coasters. It's really saddening to know that instead of making modifications to actually make the rider safer, that they took shortcuts and intentionally ignored warning signs, and worse yet, covered them up.

Kudos for coming back to set the record straight after the final report was released... most people would just gloss over their earlier video & how they'd made a few incorrect guess-timations, so bravo 👏
I think it's also worth pointing out how varied the re-certification of amusement rides is. I'm always astonished how often rides are inspected & approved by people with little experience in amusement rides or _any_ similarly engineered structures. I get that it's a niche industry, but it's no excuse for putting your city/state's stamp-of-approval on a ride that a 1st year engineering student would have questions about.
The park was clearly negligent, however the ride had been repeatedly re-certified for operation at that park.... the authorities are doing the community & reputable ride operators out there a disservice by dodging any of the responsibility for an unsafe coaster being allowed to operate.

TLC must be given to any piece of machinery no matter how simple or complex. It is especially critical as the model ages & ride manufacture defunct. $$$ should never be held over human safety. BTW the "flanged wheels" for that model acts as road + guide wheels. True If kept in proper operating range with up stop wheels (properly maintained for wear) would be just fine as long as the ride was 'trimmed' in the proper places.

As long as the ride is well maintained, it's not a problem at all. These rides were designed with that wheel setup in mind, and some gentler rides built today still use them. It's a perfectly safe design

We had a wooden roller coaster at Geauga Lake called the Big Dipper and that coaster always flew up off the tracks and came slamming down after the first hill bcuz it was fast af. It was the norm for it and it was just the way it was. Lol. Thankfully never any injuries or death.

It seems a lot of these accidents occur because parks are trying to coverup issues so they don't have to pay for extra maintenance. I wonder if there was a way to fix this issue because as greedy as it is, it seems to be human nature to sacrifice safety for money.
Could there be a maintenance industry that fixes mistakes for little cost to the amusement park or something? Or government sanctioned maintenance organization? But I guess there are so few accidents like these that cars, guns, etc are the bigger issues.

I have a question so you have e said that theme parks are so safe you have a higher chance of getting in an accident on the road then a theme park but from what I have seen you never said any thing about festivals or carnivals cuz they only have a few days to a week to put stuff up but theme parks have years to put the stuff up ever since I thought about that I have not really been to carnivals so are carnivals “safe” cuz I think they are death traps
