I think a good solution to the cheating problem is to have an accuracy over time stat that the anti-cheat monitors. And then when there’s a huge spike in accuracy after the first 2 mins, it flags them for review automatically. To avoid cheaters that lower accuracy, like for cass, there can be a FtH accuracy stat as well
But it’s a more complicated approach, they would be getting thousands of flagged games, anybody can have really good games where their aim is a lot better than usual.
It would also have to be character dependent. For example, I've been playing for 5 or 6 years, and while I can snipe widows with Torb's primary fire my accuracy with hitscans is 50% at best. There's a big difference between aiming and accuracy with hitscans versus projectiles. I understand what you're going for, but no person performs at a consistent enough level for accuracy tracking alone to be enough.
I would probably get flagged a few times for this and I'm dogwater. I'll start out matches with horrid aim then hit 5 crit kills in a row. Just happens sometimes. I know I'm far from the only one to experience this, so yeah I think they could have an overload.

Second guy didn't look like he was cheating at all. Not even walls. Team mates pinging and just looking at the spots where the enemy widow was last seen (got killed there by the hacking widow)
To determine if someone is cheating, you don't only look at what they are doing; you should look at what they aren't doing. The second cheater had horrible positioning, crosshair placement, mechanics, and awareness. You don't see that to that extent in a T500 lobby.

Which is valid in my experience of fighting against T500 Widow players and watching their kill cam/ spectate their gameplay. I got tired of reporting because it just takes up too much of my time to go back/look at the replay to make sure I'm right. Then report. Almost everyone I have reported I've gotten a report back saying that they took action so I'm pretty accurate at determining who is and is not cheating. But it just takes too much of my time. Now I only report if it's in comp and it caused a loss.

you know the later the match goes the more "data" you gather how which enemy moves which increases accuracy, i bet you are doing the same.
