Since there is currently no lore, I'd like to think this is similar to 27 hours sans but the opposite where instead of the government nuking the underground, a war was going on and nukes were being tossed around from country to country. So when frisk emerged she is not greeted by the same surface she is used to or wanted to see, instead a radioactive wasteland and the still ongoing war.
This is just my theory, not any type of lore. You are free to think about what happened and how it panned out since no lore = free interpretations from viewers
