The nurse should report A, stridor immediately if experienced by the client.
      Rationale:
Stridor is a high-pitched, wheezing sound caused by disrupted airflow. It indicates upper airway obstruction, which can be a sign of serious complications such as laryngeal edema, tracheal stenosis, or other forms of airway narrowing.
This condition can quickly lead to respiratory distress and requires prompt medical intervention to ensure the airway remains open and the patient can breathe adequately.
Occasional pink-tinged sputum (b) can be a normal finding after extubation due to minor trauma from the tube.
Respiratory rate of 24 (c) is slightly elevated but may not be immediately alarming if the patient is otherwise stable and not showing signs of severe respiratory distress.
A few basilar lung crackles in the right (d) could be due to atelectasis or residual secretions and, while needing follow-up, is not as immediately critical as stridor.
