Dr Westman, I appreciate your putting out these contents.
For years, I had followed the Dietary Guideline for “healthy” eating. Eating mostly veggies, fruit, and whole grains; from expensive Whole Food stores with lean meat only. Got my 15k steps daily.
And yet, the weight kept going up and up. Got all 5 of the metabolic syndrome conditions.
Tried so many time that “calories in calories out”, eat very low calories, still focusing on predominantly plant. I was hungry all the time, never felt good.
My weight yo-yo over the course of 3 years. None of the biomarkers improved.
Found your content 5 weeks ago trying to understand why I’m failing with these “healthy” eating.
Everything you said made a lot of sense. Switched to carb elimination.
5 weeks in, I was never hungry. Never counted calories.
Lost 26 pounds, with 20 pounds lost from fat (per iHealth scale).
My fasting glucose level dropped from 147 to 87.
My blood pressure dropped from 147/107 to 117/78
My waist circumference dropped from 46 inches to 40 inches.
My HDL went from 35 to 40
My triglycerides drop from 225 to 170
Still got some way toward becoming healthy. But I definitely see immediate improvement.
I believe if I had continued to listen to these “nutrition made simple” experts pushing predominant plant food, I would continued to get worse.

Part of the reason is that most people are not familiar with the laws of thermodynamics, and it sounds reasonable. If you take in more energy than leaves you, you have to store it. Calories are treated as being a measure of the energy your body extracts from food. As you know, that is not true, and it tells you nothing about how your body is going to use them.

I love Dr Westmans ability to break down these videos it’s very informative and helps to understand the differences of philosophy, perspective and teachings between Dr’s, diets and medical advice. What I saw in the plant based video was the Dr saying a lot of “I don’t know, I don’t see a problem but I’m not sure, I don’t do that, these studies are inconclusive etc….” it looks to me like his teaching is do what I do but I’m not really sure what I’m doing is right and no one else knows either. I suspect that’s what he has to say without clinical experience it’s all just opinion. Which is fine except exactly what Dr Westman teaches works amazingly well for me and many others. Unfortunately, Dr. Carvalho is too influenced by this n=1 experiment and he ignores the other n=500,000 or more "experiments" that strongly suggest that the keto, ketovore, or carnivore diets are far more healthy for the average person. While there are people who can consume 150 grams of carbohydrate per day with minimal ill effects, the experience of most Americans proves that high carbohydrate diets lead to poor health, weight gain, diabetes, non alcoholic fatty liver disease, dementia, polycystic ovarian syndrome, and the intensity of the symptoms of number of psychiatric conditions (depression, anxiety, Parkinson's, bipolar disorder, schizophrenia). If those 150 grams are in the form of processed foods and include sugar, grains, and seed oils, the effects are typically far worse than if the carbs are in the form of unprocessed fruits and vegetables.

I agree with your questions regarding the choice of myths. Whilst I found the lack of dogma in the presenter's approach refreshing, I was left feeling that he was dithering, or unwilling to come down on any side of the debate. Perhaps, as you pointed out, this could be down to not having an agenda, be it low-carb, keto, vegan, or - a new one for me - predominantly plant.
The plant versus animal protein argument is often overstated, and it is true that all the essential amino acids can be provided with the right combinations on a vegan diet. This, however, makes the assumption that the average consumers are nutritionally educated and aware.
When you look at recent statistics here in the UK, where 800,000 people are presenting to health professionals with malnutrition - children with rickets - nutritional illiteracy would seem to be more the order of the day.

Fructose will not show up on an A1C. For that matter, if FASTING insulin is being tested, that will not reflect the insulin levels day to day when not fasting. The video mentions the apple test was done on diabetic patients. That means they all had high insulin and high blood sugar. The whole study is pointless.

I think everyone should try a CGM at least two weeks, even if they don't have diabetes. The problem is getting people to understand what the CGM data means. I have a neighbor who thinks the purpose of CGM is to tell him when to inject more insulin and "fix it" in his words. No. The CGM is not to help you inject more insulin. The CGM is to teach you not to eat foods that set off a rise in blood glucose.
I have a friend who tried CGM at my suggestion, because he was concerned why he was putting on weight, and his belly fat is getting more. After trying it for two weeks, he showed me, saying , look! I ate rice and it only went to 9.0 mmol/Litre, and in four hours his blood sugar was back at 4.9 mmol/Litre. I tried to explain, if he keeps doing that then over time the CGM readings will get higher and higher, it will take longer and longer to return to normal after a meal, and one day his doctor will tell him he is full diabetic. A metabolically healthy person has ZERO rise in blood glucose after a meal. Their insulin works 100 percent to keep the blood glucose flat-lined.

Dr Westman, I'm in Thomasville. I've been watching you with great interest. I'm heathy, but a couple who is very close to me are not. He is seriously diabetic. His wife is under the care of atrium wake Forest heart docs trying to get in the heart transplant list. Mind you, they are mid forties. How long does it take to get an appointment with you and is referral required?

HBA1C doesn’t show fructose so for buddy to say bananas and other fruits didn’t impact A1C is false.

The other day my new primary care physician said that she thinks you have to be on insulin to get a continuous glucose monitor through insurance, at least. Is that true? I have very recently learned about the effects of oxalates on the human body, so in addition to cutting carbs as low as I can, I am also cutting high-oxalate foods, which are almost entirely plant foods.

It was true when I checked with my insurance company but maybe you have a better insurance plan.

@@susanhopemason Lol, probably not. I did end up ordering a keto-mojo to monitor my blood glucuse and ketones but it involves pricking your finger.

IN DMII, fructise may be a preferred sweetener due to its sweetness level being higher than other sugars to result in Less Sugars Overall to be consumed.

Do health effects from food apply equally to fit or unfit people ? Of course not. Most tests are done on the average person who is most likely overweight, unfit and eats junk food. That is 65% of the population. Any myth might or might not be true depending on the context of an individual's health metrics.

I love the assumption that calories in, calories out (move more, eat less) is OBVIOUSLY true, because everyone's metabolism is identical. ☹So obtuse. If that were true then everyone would either be, T1, T2 or normal and that clearly isn't so.

Calories in and calories out has a lot to do with the level of insulin resistance and the amount of insulin circulating in the blood
