Hi. Thank you for posting your query. True Adductor Canal Block (ACB) doesn’t cause any motor weakness. We are blocking only saphenous nerve proximally, and popliteal plexus block via distal spread. we use low concentration like 0.2% Ropivacaine with adjuvant dexamethasone. Nerve to Vastus Medialis (NVM) travels outside the true AC. Even if you block with low concentration LA, no issues. Again there are multiple branches supplying the VM muscle. we target NVM in distal femoral triangle block, where it is separated from Saphenous nerve by a thin fibrous septa.
What block will you do for a posterior cruciate ligament reconstruction?
We use HIPAC block.
