Every time I see the price of goods in D&D vs. what an adventuring party makes in even one outing, I wonder why the entire population doesn't become dungeon-delvers. Then something like this comes along and reminds me why most people in this setting would be perfectly happy playing it safe and farming wheat or blacksmithing.
Funny story about this monster. SPOILERS FOR MY PLAYERS AHEAD The final boss of one of my campaigns is an Adult False Hydra, but I realized that the party is very weak to flying creatures since currently 3/4 party members are melee-based. Hence I gave it a second phase where it sprouts wings and starts flying around, cause I want the final fight of the campaign not to just be a big slugfest. Fast forward to a few days ago, I found a video talking about old monsters that should be added to 5e and found out about the Shrieking Terror, so I said "well that kinda does look like a flying False Hydra! Let's use that art!". Little did I know that you were gonna cover this creature only a few days later. What a coincidence.
Imagine a Shrieking Terror corps with Mindflayers riding Shrieking Terror mounts, each commanding a whole wing of Shrieking Terrors for scouting patrols as well as hit-and-run attacks.

Maintain air superiority and put the fear of close air support into them.
I mean...why stop there? Why not have a shrieking terror that's undergone ceremorphosis? Or a mindflayer scientist that's decided to turn its own tentacles into hydra-vargouilles that are still attached to it? Or just a good ol' shrieking terror that can charm the party instead since each head is from a former bard? (also, this would be annoying to edit in, but every time you flip your hair, it'd be amazing to have delayed after-images of you doing the same thing for a weird, campy psychadelic dad vibe)

Not something you want to meet in a dark alley. It's definitely throwing me that it's only Medium size. That's just a baby hydra. Just wrong on so many levels.
Actually you kinda *do* want to meet it in a dark alley. It has crap mobility in there and you've got a much better chance of grounding and killing it...
Imagine a version of this with a False Hydra instead of a regular one. People keep mysteriously turning into monsters because of some weird curse or disease of which no one knows where it comes from. Every night people wake up from a terrible shriek somewhere in the city, only for no one to know where it came from or what caused it. They just know that the following morning, another family has been infected, slowly turning into monsters. The city slowly falls into anarchy as the only way to stop the infected from turning into monsters en masse is to execute those who are showing symptoms. Only the few who are deaf speak of a pale, winged monstority which goes around kissing people and infecting them. It would probably require some tweaking to merge the starblocks well and come up with some good lore (though evil wizard experiment is always an option), but I think it would make for a truly terrifying and dangerous plot
You have to do the 3.5 Fiend Folio monster "the Hullathion". Anything that has a special ability called "ring of puss" you know you got a great monster
A mind flyer riding a shrieking terror flies into battle holding a wand... and a bottle of levitation. It doesn't make someone levitate. It levitates. That way the mind flayer can spin it from atop its mount.
On Eberon where the Mind Flayers created these horrors I can imagine them also using their own larvae in experiments with monsters, so if they have a hold of a hydra would it take one brain tadpole to transform it, one for each head, or would you be merciful having such an abomination to be impossible. I'm sure you're picturing a Hydra with a Illithid head on each neck & able to use the mental abilities of the whole from each head. That or combine the illithid larvae with the Varghouls for flying, psychic, tentacle horrors. Both are more serious than one combination monster I saw a drawing of online, the Megoosa, part Medusa but with geese instead of snakes.
I feel like a part of this is to run all the normal precautions for boundaries for these things, cause some players may not be comfortable with their characters being forcefully kissed even if it is a bat winged head hydra.

Aberrations are clearly inspired by cosmic horror and in my opinion they should be used like cosmic horror monsters. First, put the fear of god in the hearts of the players, they should be in the Dark Souls mindset of questioning if the furniture is out ta get them, then the monster can show up.
"Its shriek is a combination of nails on a chalkboard and your parents saying they aren't mad, just disappointed in you." *takes 4d12 + 4d10 + 20d8 + 4d6 + 80 emotional damage x4*
This gives me a great idea. I'm not really a DM, I've only DMed two short one shots that were almost completely, if not entirely improvised. BUT, I have a character that I've never gotten an opportunity to use that would be a great villain to use alongside shrieking terrors. He's a yuan-ti wizard/warlock, who's obsessed with power. He became a wizard to gain power on his own, but was unsatisfied with his slow moving progress, so he made a deal with a fiend (probably a demon rather than a devil, if that fits the lore of whatever campaign he ends up in) to gain more power faster. Since shrieking terrors are used in war, his fiendish patron could order him to gather monsters to make shrieking terrors, or even make them himself.
An interesting dynamic might be that "regular" vargouilles were rounded up to make one of these Shrieking Terrors, and generally hate them, but the vargouilles that are created from the Shrieking Terror's victims are the ones that consider the creature to be aspirational. This could create a situation where the one responsible for the Shrieking Terror has used it to create more vargouilles from it's victims, so that those can be lured in by the promise of becoming another shrieking terror, which means any time they manage to get their hands on another hydra leads to their vargouilles collection steadily growing if they aren't being interfered with by a 3rd party. Meanwhile the vargouilles that aren't a result of a shrieking terror would have this attack on sight attitude towards them all; If monster in-fighting like that is uncommon as far as the party has seen before this could be an important hint towards the introduction of this threat.
If I had a nickel for every time Dungeon Dad talked about an aberration with multiple human heads attached to a lumpy body, I'd have two nickels. Which isn't a lot, but it's weird that it's happened twice.

The paltry amount of rich art about the Tarterian Depths of Carceri and its denizens (especially the demodands) is a travesty. The abyss and the nine hells are cool, but a literal prison plane consisting of an infinite amount of spheres filled with the very worst monsters and dangerous individuals the multiverse has to offer (almost rivaling the Far Realm), should get more respect from referees and dread from players than it currently does.
Could you make a playlist called the Carceri Codex and compile all your remade monsters sheets for a TERRIBLY convenient tool for a particularly upset dm?
My plan for this creature is to have the newly crowned Archfey (who is actually the eldritch deity, Y'houndeh, the wife of Nyarlathotep) have these as security drones circling her palace. The heads of the Shrieking Terror and the Vargouille will be the heads of previous Fey Court dwelling elves.

For the campaign my girlfriend and I are going to run, this would be perfect for a group working for the big bad that experiment on anything and anyone they can, including themselves and their daughter (who is the big bad in question.) No doubt in my mind they'd make something like this.
because i cannot imagin how it kisses an Aaracocra midair on max speed inmids a fierc battle and neither my players could i think i erase that action and give the effect to its attacks

I think I'd make it Large. It just doesn't make sense to have 5 heads and big wings, and only take up a 5ft square in combat.

For the campaign my girlfriend and I are going to run, this would be perfect for a group working for the big bad that experiment on anything and anyone they can, including themselves and their daughter (who is the big bad in question.) No doubt in my mind they'd make something like this.

This curse from this monster if you let it advance just before the third stage and you managed to somehow get rid of it, what happens? Does the changes continue after the curse is lifted or do they regress to before the curse was set upon said individual?
I believe based on the wording of the actual monster stats the curse would regress until it was totally gone.
