      In hx addition add h to the carbon which has more hyrodrogen in double bond this is MOR and when in the presence of hbr / peroxide make MOR riverse it is called anti-MOR or peroxide effect or kharash effect

    in the 4th step of the first example (where the first H2O molecule is introduced), where does the + charge go and why?

      Look at the carbon with the + with respect to the formal charge. When H2O is introduced, the carbocation has a steric number of 4 (4 things attached) making the formal charge 0. Similarly, oxygen has the positive charge after it is introduced because of the formal charge.
