The reason they had the scene where Sasuke fought the Hokage is because this game came out before this was adapted by the anime. They added their own original ending with this game, and took their own liberties.

Naruto has regenerated his left lung after being hit by Sasuke’s Chidori during their fight in the FV. If he could regenerate from that I’m sure he could regenerate from that attack from the Ten Tails. Neji’s death was just to add excitement, and stir the plot.
