People can be fluent even in 1 year if they have a desire to learn. I know its a hard step but you need to understand these are developed countries and if they are taking a chance to offer you a place in their medical system with good pay and great probability to be enrolled in much higher pg programmes in countries like usa , uk , germany then learning their language is the least you can do.
First step would be to study in europe so that you can avail Schengen visa. With that you have passage to nearly 28 countries. And then you need to follow proper timetable. Divide your study time and see when is the upcoming vacation. Preplan everything. I used to travel mostly in weekends . Specially to nearby eu countrie. And if you have a big gap , you can travel more.

Although you can be a part of erasmus programme that gives you opportunity to do your clinical practise in different partner countries of europe. But it will be semester wise. And if you are still interested in tourism , choose some courses that gives you opportunities for that . Being a medical student , you cant afford to travel too much , you need to divide your time .
