As of I know, those CMS 1500 claims are asked to send to hospice care directly not the hospital, these are the scenario where Physician is treating patient for their terminal illness and has direct contract with Hospice care or physician is employed by Hospice in that case you would call to get the correct hospice details and then send CMS1500 directly to hospice instead of Medicare

109 show claim was submitted to incorrect payer need to file claim to the correct payer.
CO-22 Secondary payment cannot be considered without the identity of, or payment information from, the primary payer. The information was either not reported or was illegible. The patient's care should be covered by another payer per coordination of benefits.
