आज इस वीडियो में Dr Jay Mehta , No Sperm count का इलाज कैसे करे? ( Micro TESE | Non-Obstructive Azoospermia Treatment) के बारेमें बात करेंगे। Microdissection TESE ये nonobstructive azoospermia केलिए सबसे बेहतर इलाज है।
Non obstructive azoospermia का कारण कुछ भी हो लेकिन microdissection TESE यह goldstandard treatment है और कोई भी अच्छे clinic में microTESE करने के बाद तक़रीबन 55-65% sperm retrieval मिलेगा। जैसे की हम जानते है nonobstructive azoospermia में testis के अंदर sperm production कम रहता है इसलिए यह जानना काफी महत्वपूर्ण है की microdissection से इसका इलाज कैसे हो सकता है।
MicroDissection Surgery in Non-obstructive azoospermia
-MicroTESE में testis के ऊपर छोटा सा inscision किया जाता है , इसके बाद testis पे बोहोत बड़ा camera लगाया जाता है जिसे operating microscope कहा जाता है। इसीलिए इसे Microdissection TESA ( micro TESE) कहा जाता है।
-Testis में बोहत सरे compartments होते है और उसमे seminiferrous tubules होते है जिसमे स्पर्म बनते है | -Nonobstructive azoospermia में काफी सारे seminiferrous tubules defected हुए होते है।
-Operating microscope के मदत से healthy seminiferrous tubules ढूढ़े जाते है।
-इस सर्जरी केलिए १-२ घंटे लग सकते है। इस पूरी sperm retrieval प्रक्रिया के बाद , निकाले हुए sperm को female partner के egg के साथ fuse किया जाता है। इस IVF के technique को ICSI कहा जाता है।
-Nonobstructive Azoospermia में इलाज का नाम रहता है Microdissection TESE followed by ICSI |
काफी बार लोगो के मन में ऐसा होता है की अगर डॉक्टर को यह sperm मिलने ही वाला है तो कुछ ऐसी दवाइया देदो जैसे मरीज के sperm production बढ़ जाए। शायद ही कभी डॉक्टर injections का इस्तमाल करते है जैसे की hMG + hCG के injections और latrozole tablet का इस्तमाल करते है। इन दवाइयों के मदत से यनेके less than 5 % cases में sperm production बोहोत ही कम हो तो इन दवाइयों से बढ़ाया जाता है।
Microdissection सर्जरी के बाद में पेशेंट उसी दिन घरपे जा सकते है और दूसरे दिन से मरीज काम पे जा सकता है। यह काफी safe surgery है। Average sperm retrieval rate 55-65% होता है , कभी कभी थोड़े cases में कम या ज्यादा भी हो सकता है। इन सारे patients का Micro TESA , ICSI करने के बाद test tube का success rate 15% होता है। काफी बार यह एक या दो बार भी करना पड़ता है।

I have done semen analysis on may 31st ,2021 and got the sperm count as 30 millions. But, when I go through semen analysis on 7 th august 2021, the result were zero sperm count, and i again gave a sample on 13th august 2021at same lab, same zero count result came. I am very much feared whether I have azeospermia.. And is it temperory Or permanant...
