    Hi, been on marathon watching your previous vids. Can you comment on inverse I:E ventilation for ARDS? how about the tidal volume as low as 4-6 instead of 6-8? Thanks!

      Hi Liz! Thank you again for watching and commenting with these questions. Here you go. 1) Inversed I:E ventilation places a focus on increasing MAP to enhance oxygenation by making the inspiratory phase of the breath longer than the expiratory phase. Because ARDS presents with very noncompliant lungs, their expiratory time constant is very short, therefore a long or even normal expiratory time is not necessarily needed. So essentially, we can take from E time and give to I time to maximize oxygenation and alveolar recruitment by means of the increased MAP. The practice of inversed I:E ventilation has basically lead to the development and application of APRV to help treat ARDS. 2) According to the ARDSnet protocol, we want to utilize our patient's plateau pressure to adjust tidal volume. 6 ml/kg is our goal, but if plateau pressures are greater than 30, it is recommended to decrease tidal volume by 1 ml/kg with 4 ml/kg being your minimum, in hopes of decreasing plateau pressures
