  • www.Heart-Valve-Surgery.com - With the recent FDA approval of Low-Risk transcatheter aortic valve replacement (TAVR), patients are asking important questions about the use of this non-invasive technology for the treatment of aortic stenosis (a narrow heart valve). In this video, Dr. Chris Malaisrie from Northwestern Medicine and Jim Whitney, his patient, discuss the use of surgical aortic valve replacement (SAVR) and TAVR for Jim's bicuspid aortic valve. Is this treatment available for aortic regurgitation yet?

I have this exact same problem with my bicuspid valve and will probably be having surgery soon. The VA is treating me but will probably send me to the U of Iowa. I'm seventy-seven and curious about the materials the replacement valve is made out of and how long it would last before needing to be replaced? Thank you for the clear discussion from both sides of the knife.

Did you get it done? My dad is in the ER because of this

@@MrSingh1150 Yes I had the TAVR in July at the University of Iowa, and the results were absolutely amazing. One over night in the hospital, no real pain, told to take it easy for a few weeks. The renewed energy and stamina I have gained back can only be described as awesome.

3:27 Does this mean the patient is awake during the procedure? Sounds scary

Hi Nick, We understand your concern about twilight anesthesia during TAVR. That said, you might want to review this research about the utility of conscious sedation during transcatheter aortic valve replacement - www.acc.org/latest-in-cardiology/articles/2020/03/16/15/06/conscious-sedation-associated-with-improved-tavr-outcomes-vs-general-anesthesia-acc-2020. Hope that helps!

Does Dr. Chris use ridgid fixation of the sternum for aortic stenotic valve replacement if its done thru an opened chest? And, does Dr. Chris recommend use of a brace like qualibreath to minimize pain when coughing, sneezing and laughing after a sternotomy?

Hi, Great question. Unfortunately, we do not know the specific techniques of each heart surgeon that we work with. Here is a link to more information about Dr. Malaisrie. If you like, you can call his office to learn more about whether or not Dr. Malaisrie uses rigid sternal fixation.

What type of valve did you use?

SIMPLE QUESTION : If you have a first open heart surgery to replace a defective or bicuspide aortic valve at age 45 -50 and choose a cow valve can you avoid a second open heart surgery in the future by having a TAVR procedure to replace your first cow valve ? (assuming surgery is the only first procedure option due to a dilated aorta ) Thank You and keep the good work

@@HeartValveSurgery Thanks for the quick response .Just read the article..very interesting.I've also found out that it's possible to have multiple TAVR procedures after only one open heart surgery (great option for younger patients )

@Covert Puppytwo Guess what ? I already had the surgery and had the resilia valve put in.Hope it lasts longer than the other valves and I wait at least 15 years for the TAVR procedure

@Covert Puppytwo This valve was released in 2018 so the is not much data on it yet.But the manufacturer claims it lasts longer than other tissue valves.I really hope you are right and lasts over 20 years.Good luck with your surgery & keep us posted !

I have exact same issue. My problem is I'm 59 and I have had 2 insurance denials. Which is very difficult to understand as this technology is 15 years old and over 300K operations form what I've read. We think TAVR is the way to go but how to pay for it? What would you do? Obviously I'm looking at financing but what would we do if I could not mortgage my house? And in 5 years its all covered. Looking for another outlier for advice. I think my choices are between mechanical w insurance or TAVR completely out of pocket. I e paying it back over 20 years vs the mechanical risks of open heart surgery and non invasive TAVR. Thanks for any suggestions.
