The 2 facets for his ult are extremely niche--close to useless. They don't do anything until Lv6 when you actually get his ult.
Blood Bath slows enemies next to the target but they rarely group together until late game or you have a Dark Seer in your team. Nothl Transfusion heals your allies next to the target, but those occasions are rare or situational; for example, if your ally is being chased then you should also probably run away, or if they initiated and need heal, that should the job of a support.
Choosing either Blood Bath or Nothl Transfusion is like playing without a facet for 5 mins at best & upto 10 mins in a bad game.
Incendiary is risky because the cost is 4% MAX hp instead of current hp but it's the best pick of all 3 for 3 reasons.
1. It's useful because you need Burning Spear for dmg & harass from the start.
2. The facet makes it like you have Lv2 Burning Spears at Lv1, lets you give some lvls to Inner Fire, since instant dmg is better than dmage over time.
3. More Burning Spear damage is good for farming.
4. It scales with target HP, especially late game.
