Carcinoma of Esophagus-
Introduction-
Most common in 6th decade
Common in Male
Most common symptom-Dysphagia
Common Site-
Cervical & Thoracic Oesophagus-
Squamous epithelium- Squamous cell Carcinoma- common in India 90%
Abdominal Oesophagus(2-3cm)-
Columnar Epithelium- Adenocarcinoma
In Western countries
Precancerous Condition-
Achalasia Cardia (failure of LES relaxation due to ganglion cell)
Reflux Esophagitis
Plummer Vinson Syndrome (UES)
Corrosive Liquid intake
Fungal Contamination of Food mainly in China.
Diet, Deficiency(Vit. A, C)
Chronic smoking
Alcohol, Tobacco, Cold drinks
Clinical feature-
Progressive Dysphagia (18 month) (2/3rd lumen occluded)
Odynophagia (in advance Condition)
hematemesis
Regurgitation
Hiccough, due to phrenic nerve involvement
Loss of Appetite & weight
Substernal Pain or in abdomen
Left Supraclavicular lymph node may be palpable.
Broncho- Pneumonia
Hoarseness of voice (if Recurrent Laryngeal nerve involve)
Metastasis
Investigation-
Hb%, ESR, CBC, LFT
Barium meal X-ray (chest)- aspiration pneumonia
CT-Scan (95% accuracy)
Gastroscopy
Bronchoscopy- see invasion in upper 1/3rd carcinoma oesophagus
Esophagoscopy- see lesion, extent & type.
Endoscopy
Biopsy- for confirmation
Treatment-
When absent of nodal spread-
Radial Esophagectomy
If 5 or less nodes are involve-
Radial Esophagectomy + Radiotherapy + Chemotherapy
If more then 5 nodes involve-
Poor prognosis
